Description

Indications
Ace is prescribed for the relief of fever, common cold, influenza, headaches, toothaches, earaches, body aches, muscle pain, nerve pain, menstrual cramps, sprains, colic pain, back pain, post-operative pain, postpartum pain, inflammatory pain, and pain following vaccinations in children. It is also recommended for rheumatic and osteoarthritic pain, as well as joint stiffness.

Pharmacology
Paracetamol exerts its analgesic effect by blocking pain impulse generation at a peripheral level. It lowers fever by acting on the hypothalamic heat-regulating center. Its mild anti-inflammatory action is associated with inhibiting prostaglandin synthesis within the central nervous system (CNS).

Paracetamol (Acetaminophen) primarily acts in the CNS by increasing the pain threshold through the inhibition of cyclooxygenase (COX-1, COX-2, and COX-3), enzymes involved in prostaglandin synthesis. Unlike NSAIDs, it does not inhibit cyclooxygenase in peripheral tissues, which explains its lack of peripheral anti-inflammatory effects. Unlike aspirin, which irreversibly blocks COX enzymes, acetaminophen indirectly inhibits COX, with reduced efficacy in high-peroxide environments such as platelets and immune cells. Studies suggest it selectively inhibits COX-3, a variant enzyme, but its exact mechanism remains under investigation. The antipyretic effects result from its direct action on hypothalamic heat-regulating centers, leading to peripheral vasodilation and sweating.

Dosage & Administration
Tablet:

  • Adults: 1-2 tablets every 4 to 6 hours, not exceeding 4 g (8 tablets) per day.
  • Children (6-12 years): ½ to 1 tablet 3 to 4 times daily. Long-term use should not exceed 2.6 g per day.

Extended-Release Tablet:

  • Adults & Children (12 years and above): Two tablets every 6 to 8 hours (maximum of 6 tablets in 24 hours). Tablets must be swallowed whole without crushing.

Syrup/Suspension:

  • Children under 3 months: 10 mg/kg body weight (reduce to 5 mg/kg in jaundiced infants) 3 to 4 times daily.
  • 3 months to below 1 year: ½ to 1 teaspoonful 3 to 4 times daily.
  • 1-5 years: 1-2 teaspoonfuls 3 to 4 times daily.
  • 6-12 years: 2-4 teaspoonfuls 3 to 4 times daily.
  • Adults: 4-8 teaspoonfuls 3 to 4 times daily.

Suppository:

  • Children 3-12 months: 60-120 mg, 4 times daily.
  • Children 1-5 years: 125-250 mg, 4 times daily.
  • Children 6-12 years: 250-500 mg, 4 times daily.
  • Adults & Children over 12 years: 0.5-1 g, 4 times daily.

Paediatric Drops:

  • Up to 3 months: 0.5 ml (40 mg)
  • 4 to 11 months: 1.0 ml (80 mg)
  • 1 to 2 years: 1.5 ml (120 mg). Do not exceed 5 doses per day for a maximum of 5 days.

IV Infusion:

  • Adults & adolescents (≥50 kg): 1000 mg every 6 hours or 650 mg every 4 hours (max 4000 mg/day).
  • Adults & adolescents (<50 kg): 15 mg/kg every 6 hours or 12.5 mg/kg every 4 hours (max 75 mg/kg/day).
  • Children (2-12 years): 15 mg/kg every 6 hours or 12.5 mg/kg every 4 hours (max 75 mg/kg/day).

Interactions
Patients using barbiturates, tricyclic antidepressants, or alcohol may have a reduced ability to metabolize high doses of Ace. Alcohol consumption increases the risk of liver toxicity. Long-term use of anticonvulsants or oral contraceptives can induce liver enzymes, lowering the effectiveness of Ace.

Contraindications
Ace should not be used in individuals with known hypersensitivity to Paracetamol.

Side Effects
Side effects are generally mild but may include blood disorders such as thrombocytopenia, leucopenia, pancytopenia, neutropenia, and agranulocytosis. Occasional cases of pancreatitis, skin rashes, and allergic reactions have been reported.

Pregnancy & Lactation
Studies have shown no adverse effects when used in recommended doses during pregnancy. Paracetamol is excreted in breast milk in insignificant amounts. However, medical advice should always be followed.

Precautions & Warnings
Caution is advised when administering Ace to patients with severe renal or hepatic impairment. Overdosing increases the risk of severe liver damage, particularly in individuals with liver disease or alcohol dependency. Avoid using multiple Paracetamol-containing products concurrently. Life-threatening anaphylactic reactions have been reported; discontinue use immediately if symptoms of hypersensitivity appear.

Use in Special Populations

  • Pediatric Use: Safe and effective in children aged 2 years and older.
  • Geriatric Use: No significant differences in effectiveness or safety between elderly and younger patients.
  • Hepatic Impairment: Contraindicated in severe liver disease; dose adjustment may be necessary in mild impairment.
  • Renal Impairment: Dose adjustment required for severe renal impairment (creatinine clearance <30 ml/min).

Overdose Effects
Severe overdose (≥10 g in adults or ≥5 g in individuals with risk factors) may cause liver damage. Early symptoms include pallor, nausea, vomiting, and abdominal pain, with liver damage manifesting within 12-48 hours. Severe poisoning can lead to hepatic failure, metabolic acidosis, and death. Immediate treatment with activated charcoal and N-acetylcysteine is essential if overdose occurs.
