Pharmacology & How Ace Works:
Ace (Paracetamol) works primarily in the central nervous system (CNS), increasing the pain threshold by inhibiting enzymes known as cyclooxygenases (COX). This helps prevent the generation of pain signals, providing significant relief from various types of pain. Additionally, Ace acts on the hypothalamus in the brain to regulate body temperature, effectively reducing fever.
Unlike NSAIDs (Non-Steroidal Anti-Inflammatory Drugs), Paracetamol does not possess anti-inflammatory properties in peripheral tissues. This makes Ace a gentler option for those who require pain relief without the gastrointestinal side effects associated with NSAIDs. Ace’s ability to relieve both pain and fever without causing significant stomach discomfort makes it ideal for long-term use in appropriate doses.
Dosage & Administration:
Ace is available in multiple forms to cater to different age groups and needs. Always follow your healthcare provider’s instructions for correct dosage.
- Tablet (Standard Release):
- Adults: 1-2 tablets every 4-6 hours, up to a maximum of 4 gm (8 tablets) per day.
- Children (6-12 years): ½ to 1 tablet 3-4 times daily, not exceeding 2.6 gm/day for long-term use.
- Tablet (Extended Release):
- Adults & Children (over 12 years): 2 tablets every 6-8 hours, with a maximum of 6 tablets in 24 hours. (Do not crush the tablet.)
- Syrup/Suspension:
- Children under 3 months: 10 mg/kg body weight, 3-4 times daily (reduce to 5 mg/kg if jaundiced).
- Children 3 months to 1 year: ½ to 1 teaspoonful, 3-4 times daily.
- Children 1-5 years: 1-2 teaspoonfuls, 3-4 times daily.
- Children 6-12 years: 2-4 teaspoonfuls, 3-4 times daily.
- Adults: 4-8 teaspoonfuls, 3-4 times daily.
- Suppository:
- Children 3-12 months: 60-120 mg, 4 times daily.
- Children 1-5 years: 125-250 mg, 4 times daily.
- Children 6-12 years: 250-500 mg, 4 times daily.
- Adults & children over 12 years: 500 mg-1 gm, 4 times daily.
- IV Infusion (Hospital Use):
- Adults & adolescents (≥50 kg): 1000 mg every 6 hours, or 650 mg every 4 hours (maximum daily dose of 4000 mg).
- Children (2-12 years): 15 mg/kg every 6 hours, or 12.5 mg/kg every 4 hours (maximum daily dose of 75 mg/kg).
Note: Always follow the doctor’s instructions for dosage and administration to ensure safety and efficacy.
Side Effects:
Ace is generally well-tolerated, but as with any medication, side effects may occur. Possible mild side effects include:
- Skin rashes or allergic reactions
- Stomach discomfort or nausea
- Rare hematological reactions like thrombocytopenia, neutropenia, or pancytopenia
If you experience any severe reactions like swelling, difficulty breathing, or unusual bruising, discontinue use and consult a healthcare professional immediately.
Precautions:
- Liver & Kidney Impairment: Caution should be exercised in patients with liver or kidney conditions. Avoid excessive alcohol consumption as it can increase the risk of liver damage.
- Overdose Risk: Do not exceed the recommended daily dose to avoid the risk of hepatotoxicity (liver damage), especially in those with pre-existing liver conditions.
- Pregnancy & Lactation: Ace is generally considered safe for use during pregnancy when taken at recommended doses. Paracetamol is excreted in breast milk, but not in clinically significant amounts. Always consult your doctor before use.
Interaction with Other Drugs:
- Barbiturates, tricyclic antidepressants, and alcohol can interfere with the metabolism of Ace and may increase the risk of liver toxicity.
- Anticonvulsants and oral contraceptives can reduce the effectiveness of Ace by inducing liver enzymes.
- Avoid taking multiple medications containing paracetamol to prevent overdose.
Storage:
Keep Ace in a cool, dry place away from light and heat. Ensure the medicine is stored out of reach of children to avoid accidental ingestion.
