Indications
Flupentixol and Melitracen tablets are prescribed for the treatment of:

• Anxiety disorders
• Depression
• Apathy
• Psychogenic depression
• Depressive neuroses
• Masked depression
• Psychosomatic conditions accompanied by anxiety and apathy
• Menopausal depression
• Dysphoria and depression in individuals with alcohol or substance dependence

Pharmacology
This formulation combines two well-established active ingredients: Flupentixol, a neuroleptic with inherent anxiolytic and antidepressant properties when administered in small doses, and Melitracen, a bipolar thymoleptic known for its activating effects at low doses. Together, they provide a synergistic effect, delivering antidepressant, anxiolytic, and activating benefits.

After oral administration, peak serum concentrations are reached in approximately 4 hours for both Flupentixol and Melitracen. The biological half-life of Flupentixol is around 35 hours, while Melitracen has a half-life of approximately 19 hours. The combination does not significantly alter the pharmacokinetic properties of either component.

Dosage & Administration

• Adults: Typically, 2 tablets daily—one in the morning and one at noon. In severe cases, the morning dose may be increased to 2 tablets.
• Elderly Patients: 1 tablet in the morning.
• Maintenance Dose: Generally, 1 tablet in the morning. For patients experiencing severe restlessness or insomnia, additional treatment with a sedative may be necessary during the acute phase.

Drug Interactions

• This tablet may enhance the effects of alcohol, barbiturates, and other central nervous system (CNS) depressants.
• Concurrent use with monoamine oxidase (MAO) inhibitors may trigger hypertensive crises.

• Thymoleptics may enhance the effects of adrenaline and noradrenaline.

Contraindications
This medication should not be used in:

• Individuals with untreated narrow-angle glaucoma
• Cases of acute alcohol, barbiturate, or opioid intoxication
• Patients who have taken an MAO inhibitor within the past two weeks
• Individuals who are highly excitable or hyperactive, as the activating properties of this medication may exacerbate such conditions

Side Effects
At the recommended dosage, side effects are uncommon. However, some patients may experience transient restlessness or insomnia.

Use in Pregnancy & Lactation
It is advised to avoid using this medication during pregnancy and lactation unless specifically recommended by a healthcare professional.

Precautions & Warnings

• If the patient has previously been treated with sedative tranquilizers, these should be gradually withdrawn.

Overdose Effects
In cases of overdose, symptoms related to Melitracen intoxication, particularly anticholinergic effects, are predominant. In rare instances, extrapyramidal symptoms associated with Flupentixol may appear.

Management of Overdose:

• Immediate gastric lavage and administration of activated charcoal are recommended.
• Supportive treatment should focus on maintaining respiratory and cardiovascular function.
• Epinephrine (adrenaline) must not be administered.
• Diazepam may be used for convulsions, while biperiden can help manage extrapyramidal symptoms.
