Indications

Adlina is prescribed for adults with type 2 diabetes mellitus to help regulate blood sugar levels.

  • As monotherapy: Recommended for patients whose blood sugar levels are not adequately controlled through diet and exercise alone and for whom metformin is unsuitable due to intolerance or contraindications related to kidney impairment.

Pharmacology

Linagliptin, the active ingredient in Adlina, is a dipeptidyl peptidase-4 (DPP-4) inhibitor. This enzyme plays a role in breaking down incretin hormones such as GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). By inhibiting DPP-4, Linagliptin increases the levels of these active incretin hormones, leading to enhanced insulin release from pancreatic beta (β) cells in a glucose-dependent manner while reducing glucagon secretion from pancreatic alpha (α) cells. This process helps to maintain blood sugar levels within a controlled range.

Dosage & Administration

  • The recommended dose of Linagliptin is 5 mg once daily.
  • When used with metformin, the metformin dosage should remain unchanged, and Linagliptin should be taken alongside it.
  • If combined with a sulfonylurea, a lower dose of the sulfonylurea may be considered to minimize the risk of hypoglycemia.
  • Patients with renal impairment: No dose adjustment is required.
  • Adlina can be taken with or without food at any time of the day.

Drug Interactions

  • Adlina is a weak to moderate inhibitor of the CYP3A4 enzyme but does not significantly affect other CYP enzymes.
  • Clinical studies have shown that Adlina does not have any significant impact on the pharmacokinetics of commonly used medications such as metformin, glyburide, simvastatin, warfarin, digoxin, or oral contraceptives.
