Indications:

Adlinameg is prescribed as an adjunct to diet and exercise for improving glycemic control in adults with type 2 diabetes mellitus. It is specifically recommended when treatment with both Linagliptin and Metformin Hydrochloride is suitable.

Pharmacology:

Linagliptin: Linagliptin works by inhibiting the enzyme DPP-4 (dipeptidyl peptidase-4). DPP-4 normally breaks down incretin hormones such as GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic peptide). By inhibiting this enzyme, Linagliptin increases the levels of these active incretin hormones. This, in turn, helps stimulate the release of insulin from the pancreatic beta (β) cells in a glucose-dependent manner and reduces glucagon secretion from the pancreatic alpha (α) cells.

Metformin Hydrochloride: Metformin, a biguanide class oral antihyperglycemic drug, is commonly used to manage type 2 diabetes. It reduces both basal and postprandial glucose levels. Unlike sulfonylureas, Metformin does not cause hypoglycemia. It primarily works by reducing hepatic glucose production, decreasing the absorption of glucose from the intestines, and enhancing insulin sensitivity, which increases the uptake and utilization of glucose by peripheral tissues.

Dosage & Administration:

Linagliptin & Metformin Immediate Release Tablet:
The dose should be tailored based on both efficacy and tolerability. The maximum recommended dose is 2.5 mg of Linagliptin and 1000 mg of Metformin Hydrochloride, taken twice daily with meals. It’s important to gradually escalate the dose to minimize gastrointestinal side effects commonly associated with Metformin.

Starting Dose:
For patients not currently on Metformin, begin with 2.5 mg of Linagliptin and 500 mg of Metformin Hydrochloride, taken twice daily.
For those already using Metformin, start with 2.5 mg of Linagliptin and the same dose of Metformin, taken twice daily.
For patients on Linagliptin and Metformin separately, switching to this combination tablet with equivalent doses of both components is recommended.
Linagliptin & Metformin Extended-Release Tablet:
The dosage should be customized based on efficacy and tolerability, not exceeding 5 mg of Linagliptin and 2000 mg of Metformin Hydrochloride per day. This formulation is taken once daily with a meal.

Starting Dose:
For those not yet using Metformin, initiate treatment with 5 mg of Linagliptin and 1000 mg of Metformin Hydrochloride in extended-release form, once daily.
For patients already on Metformin, begin with 5 mg of Linagliptin and the corresponding dose of Metformin, taken once daily.
For those switching from the immediate-release tablet form, the extended-release tablet should be taken once daily with the equivalent doses.
Note: Take 5 mg of Linagliptin with 1000 mg of Metformin Hydrochloride as a single tablet daily. If using 2.5 mg of Linagliptin, take two tablets once daily.

Important Note:
Use Adlinameg as directed by a healthcare provider.

Drug Interactions:

Cationic Drugs: Medications like amiloride, digoxin, morphine, and others that are cleared by the kidneys may interact with Metformin. These drugs could compete for renal tubular transport, requiring careful monitoring and dose adjustment.

Carbonic Anhydrase Inhibitors: Drugs like topiramate may lead to metabolic acidosis when used with Adlinameg. Caution is advised as the risk of lactic acidosis may increase.

Inducers of P-glycoprotein and CYP3A4: Medications like rifampin that induce P-gp or CYP3A4 enzymes may lower Linagliptin effectiveness. When these drugs are needed, alternative treatments should be considered.

Drugs Affecting Glycemic Control: Certain medications can increase blood sugar levels, potentially affecting Adlinameg’s ability to control glycemia. Such drugs include diuretics, corticosteroids, thyroid medications, oral contraceptives, and others. Monitoring and adjustments are required to maintain appropriate blood sugar control.

Contraindications:

Adlinameg should not be used in patients with renal impairment, as Metformin is primarily excreted through the kidneys. The risk of accumulation and lactic acidosis increases with kidney dysfunction.
Acute or chronic metabolic acidosis (e.g., diabetic ketoacidosis) or hypersensitivity to either Linagliptin or Metformin Hydrochloride also contraindicates the use of this combination.
Side Effects:

The most common side effects include nasopharyngitis (inflammation of the nasal passages) and diarrhea. Hypoglycemia may occur more frequently when Adlinameg is combined with sulfonylureas.

Pregnancy & Lactation:

There is limited data on the use of this combination in pregnant women. Adlinameg should only be used during pregnancy when absolutely necessary, and with caution in breastfeeding women.

Precautions & Warnings:

Lactic Acidosis: This condition is rare but can occur, especially in patients with serious underlying conditions like heart or kidney problems. If symptoms such as abdominal pain, fatigue, rapid breathing, and muscle cramping occur, discontinue the drug immediately.
Pancreatitis: Cases of acute pancreatitis have been reported post-marketing. If pancreatitis is suspected, stop the medication immediately.
Vitamin B12: Long-term use of Metformin may decrease Vitamin B12 levels, so periodic blood tests are advised.
Surgical Procedures: Temporarily stop Adlinameg before major surgery or diagnostic procedures that require fasting or the use of contrast dyes.
Overdose Effects:

Linagliptin: In clinical trials, overdoses up to 600 mg (120 times the recommended daily dose) did not result in dose-dependent adverse effects.
Metformin: Overdose with Metformin, especially doses greater than 50 grams, can lead to lactic acidosis in some cases, although hypoglycemia is rare.
