Description
Indications
Ambrox is used to treat:
- Productive cough
- Acute and chronic inflammatory conditions of the upper and lower respiratory tracts associated with thick mucus, such as acute and chronic bronchitis
- Inflammatory diseases of the rhinopharyngeal tract, including laryngitis, pharyngitis, sinusitis, and rhinitis, when accompanied by thick mucus
- Asthmatic bronchitis and bronchial asthma with thick sputum
- Bronchiectasis
- Chronic pneumonia, among others

Pharmacology
Ambroxol is the active metabolite of bromhexine and has been shown to have a more potent bronchosecretolytic effect than bromhexine itself. It works by improving sputum rheology through a hydrating mechanism, which liquefies mucus in the respiratory tract and aids in its expectoration, thus reducing shortness of breath. Additionally, Ambroxol stimulates the production of surfactant phospholipids by alveolar cells, helping lower surface tension in the alveoli. It also reduces bronchial hyperactivity. Ambroxol has anti-inflammatory properties due to its inhibitory effects on cellular cytokines and arachidonic acid metabolites. In patients with chronic obstructive pulmonary disease (COPD), it traditionally enhances airway patency.
Dosage & Administration
The recommended dosage (preferably after meals) is as follows:

Pediatric Drops:
- 0-6 months: 0.5 ml, twice daily
- 6-12 months: 1 ml, twice daily
- 1-2 years: 1.25 ml, twice daily

Syrup:
- 2-5 years: 2.5 ml (½ teaspoon) 2-3 times daily
- 5-10 years: 5 ml (1 teaspoon) 2-3 times daily
- 10 years and adults: 10 ml (2 teaspoons) 3 times daily

Sustained Release Capsules:
- Adults and children over 12 years: 1 capsule, once daily

Interactions
Ambrox should not be used simultaneously with antitussives (e.g., Codeine), as the phlegm liquefied by Ambrox might not be expectorated properly.
Contraindications
Ambrox is contraindicated in individuals with known hypersensitivity to Ambroxol or Bromhexine.
Side Effects
While most people tolerate Ambrox well, some may experience occasional gastrointestinal side effects such as epigastric pain or a feeling of fullness in the stomach. Rarely, allergic reactions like skin rashes, urticaria, or angioneurotic edema may occur.
Pregnancy & Lactation
Teratogenic and fetal toxicity studies show no harmful effects of Ambroxol. However, it is advisable to avoid its use during pregnancy, particularly in the first trimester. The safety of Ambroxol during lactation has not been established.
Precautions & Warnings
Ambrox should be used with caution in patients with gastric or duodenal ulcers or those with convulsive disorders. Caution is also recommended for patients with hepatic or renal insufficiency.
