Description
Indications
This medication is indicated for the following conditions:
- Essential hypertension
- Co-existing angina pectoris and hypertension
- Post-myocardial infarction (MI) patients
- Refractory angina pectoris in patients where nitrate therapy has proven ineffective

Pharmacology
This product is a fixed-dose combination of Amlodipine and Atenolol.

Amlodipine: A dihydropyridine calcium channel blocker that inhibits the influx of calcium ions into vascular smooth muscle and cardiac muscle. It is more effective in vascular smooth muscle than cardiac muscle. As a peripheral vasodilator, Amlodipine works by reducing peripheral vascular resistance, which helps lower blood pressure. It also decreases coronary vasoreactivity and reduces cardiac workload by decreasing afterload.

Atenolol: A cardioselective beta-blocker, with selectivity depending on the dosage. Atenolol lowers blood pressure by reducing cardiac output, lowering plasma renin activity, and decreasing sympathetic outflow from the central nervous system. It also reduces myocardial oxygen demand through its negative inotropic and chronotropic effects.
Dosage & Administration
The recommended dosage is one tablet of Amlodipine (5 mg) and Atenolol (25 mg) once daily. If necessary, the dose may be increased to two tablets daily (5/25 mg each) or as directed by a healthcare provider. Dosage should be tailored to the individual patient’s needs.

Interactions
- Disopyramide: Atenolol may decrease the clearance of disopyramide by 20%, leading to potential additive negative inotropic effects on the heart.
- Ampicillin: Doses of 1 gram and above may reduce Atenolol levels.
- Oral antidiabetics and insulin: Beta-blockers like Atenolol may reduce tissue sensitivity to insulin and inhibit insulin secretion. However, Atenolol has a lower potential for these actions compared to other beta-blockers.
Contraindications
This medication should not be used in patients who:
- Are hypersensitive to either Amlodipine or Atenolol
- Have sinus bradycardia, second-degree or higher heart block, cardiogenic shock, hypotension, congestive heart failure, or poor left ventricular function
Side Effects
The combination of Amlodipine and Atenolol is generally well tolerated. Common side effects may include:
- Fatigue
- Headache
- Edema
- Nausea
- Drowsiness
- Anxiety
- Depression
Pregnancy & Lactation
The use of this combination during pregnancy should only occur if the expected benefit outweighs the potential risk to the fetus. It is contraindicated in nursing mothers. If necessary, breastfeeding should be discontinued during the treatment.
Precautions & Warnings
- Bronchospasm: Caution should be exercised when administering this medication to patients with airway obstruction.
- Renal impairment: This combination can be used in patients with renal impairment. However, caution is advised if creatinine clearance is less than 30 ml/min, as Atenolol may accumulate.
- Hepatic impairment: In patients with severe liver impairment, use with caution, as Amlodipine’s elimination half-life may be prolonged.
- Drug withdrawal: Sudden discontinuation of this medication should be avoided, especially in patients with coronary heart disease, which may not always be diagnosed. Any cessation of the medication should be gradual and under medical supervision.
Overdose Effects
Although overdose symptoms are not fully documented, it may lead to hypotension and, in rare cases, congestive heart failure. If an overdose is suspected, unabsorbed medication can be removed through gastric lavage or activated charcoal. Symptomatic treatment is recommended.
