Indications
Anadol is used for the management of moderate to severe pain. It is effective in treating various types of pain, including:

  • Postoperative pain
  • Colic and spastic pain
  • Cancer-related pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology
Tramadol, the active ingredient in Anadol, is a centrally acting synthetic analgesic. It works by inhibiting the reuptake of neurotransmitters, such as serotonin and noradrenaline, which alters pain transmission. The analgesic effect is achieved through stimulation of mu-opioid receptors and indirect modulation of central monoaminergic inhibitory pathways, thus helping to alleviate pain.


Dosage and Administration

Capsule or Tablet Form:

  • Usual dosage: 50-100 mg every 4-6 hours, depending on pain severity.
  • Acute pain: An initial dose of 100 mg is recommended.
  • Chronic pain: Start with 50 mg, and adjust subsequent doses to 50-100 mg every 4-6 hours.
  • Maximum daily dose: Do not exceed 400 mg per day.

Sustained Release (SR) Capsule or Tablet:

  • Take one SR capsule or tablet every 12 hours (e.g., one in the morning, one in the evening).
  • The total daily dosage should not exceed 400 mg.
  • The exact number of capsules will depend on pain severity but should not be taken more frequently than every 12 hours.

Injection Form:

  • For general pain management, administer 50-100 mg every 4-6 hours via intramuscular (IM) or intravenous (IV) injection.
  • For postoperative pain, begin with 100 mg, followed by 50 mg every 10-20 minutes, as necessary, up to a maximum of 250 mg in the first hour.
  • After the first hour, doses of 50-100 mg can be repeated every 4-6 hours, not exceeding a total of 600 mg per day.

Suppository Form:

  • Administer 100 mg Tramadol rectally every 6 hours for adults.
  • In general, the total daily dose should not exceed 400 mg (4 suppositories), but for more severe pain, such as cancer pain or intense postoperative pain, higher doses may be required.

Note: Always consult a healthcare provider to determine the appropriate dose for your condition.


Interaction
Anadol has minimal interactions with other drugs; however, monoamine oxidase (MAO) inhibitors should not be used in combination with Anadol. Additionally, combining Anadol with carbamazepine can significantly increase its metabolism, requiring an adjustment in dosage.


Contraindications
Tramadol should not be used by individuals who are allergic to the drug. It is also contraindicated in cases of acute intoxication with alcohol, hypnotics, central analgesics, opioids, or psychotropic drugs.

Common side effects may include:

The safety of Tramadol during pregnancy has not been established. Since the drug crosses the placenta, it should only be used during pregnancy if the potential benefits outweigh the risks to the fetus. Tramadol is also present in breast milk, so it should be avoided during breastfeeding.


Precautions and Warnings

  • Respiratory Depression: Large doses of Anadol, especially when combined with anesthetics or alcohol, can cause respiratory depression. It should be used cautiously in individuals at risk for respiratory problems.
  • Opioid Dependence: Anadol is not recommended for patients who are dependent on opioids.
  • CNS Depressants: Caution is needed when using Anadol with other CNS depressants (e.g., alcohol, opioids, anesthetics, phenothiazines, sedatives).
  • MAO Inhibitors: Anadol should be used cautiously in patients taking MAO inhibitors due to the drug’s effects on serotonin and norepinephrine reuptake.
  • Intracranial Pressure or Abdominal Conditions: Anadol should be used with caution in patients with increased intracranial pressure, head injuries, or acute abdominal conditions.

Use in Special Populations

  • For children aged 1 year and above, Anadol can be administered at a dose of 1-2 mg/kg of body weight.
  • Suppositories (100 mg) should not be given to children or adolescents under the age of 14 years.
  • 100 mg SR Capsules have not been tested in children, and their safety and efficacy have not been established for pediatric use.
