Pharmacology
This medication contains two active ingredients: Flupentixol, a neuroleptic with anxiolytic and antidepressant effects when used in small doses, and Melitracen, a bipolar thymoleptic with activating properties at low doses. Together, these compounds provide a balanced antidepressant, anxiolytic, and activating effect.

After oral administration, both Flupentixol and Melitracen reach peak serum concentration in approximately 4 hours. The biological half-life of Flupentixol is around 35 hours, while Melitracen has a half-life of about 19 hours. The combination of these two substances does not alter their individual pharmacokinetic properties.

Dosage & Administration

  • Adults: Typically, 2 tablets daily—one in the morning and one at noon. In severe cases, the morning dose may be increased to 2 tablets.
  • Elderly patients: Usually, 1 tablet in the morning.
  • Maintenance dose: 1 tablet in the morning. If the patient experiences insomnia or severe restlessness, an additional sedative may be recommended during the acute phase.

Drug Interactions

  • May enhance the effects of alcohol, barbiturates, and other central nervous system (CNS) depressants.
  • Concurrent use with MAO inhibitors may cause hypertensive crises.

  • Thymoleptics can enhance the effects of adrenaline and noradrenaline.

Contraindications
This medication should not be used in patients with:

  • Acute intoxication from alcohol, barbiturates, or opioids
  • Recent use of MAO inhibitors (within two weeks)
  • High excitability or hyperactivity, as its activating effect may intensify these symptoms

Side Effects
When taken at the recommended dosage, side effects are rare but may include temporary restlessness and insomnia.

Precautions & Warnings

  • If the patient has previously been treated with sedative tranquilizers, these should be discontinued gradually.

Overdose Management
In case of overdose, symptoms related to Melitracen toxicity (primarily anticholinergic effects) are more prominent. In some cases, extrapyramidal symptoms due to Flupentixol may also appear.

Treatment includes:

  • Symptomatic and supportive care
  • Gastric lavage as soon as possible, followed by activated charcoal administration
  • Support for respiratory and cardiovascular functions
  • Epinephrine (adrenaline) should NOT be used
  • Convulsions may be managed with diazepam, while extrapyramidal symptoms can be treated with biperiden
