Description

Indications:
Bondrova is indicated for:

• Treating osteoporosis in women, particularly postmenopausal women.
• Preventing osteoporosis in women, especially postmenopausal women.
• Preventing and treating osteoporosis in men.

Diagnosis and Treatment of Osteoporosis:
Osteoporosis is confirmed by low bone mass (T-score <-2.0 SD) alongside a history of osteoporotic fractures, or a T-score <-2.5 SD without prior fractures.

Pharmacology:
Bondrova contains Ibandronic Acid, a potent bisphosphonate that inhibits bone resorption. It prevents bone loss caused by hormonal changes, tumors, or other factors. In animal studies, Ibandronic Acid significantly increased bone mass without affecting mineralization, even at high doses. Its selective action on bone tissue stems from its strong affinity for hydroxyapatite, the mineral component of bones.

In postmenopausal women, Bondrova reduces elevated bone turnover rates to premenopausal levels, promoting a gradual increase in bone mass. Regular use lowers bone resorption markers, enhances bone mineral density (BMD), and reduces fracture risk.

Dosage and Administration:

• Take one 150 mg tablet once a month, preferably on the same date each month.
• Consume the tablet 60 minutes before the first meal, drink (other than water), or any other medication.
• Swallow the tablet whole with a full glass of plain water (180-240 ml) while sitting or standing upright. Avoid lying down for 60 minutes after ingestion.
• Do not chew or suck the tablet to prevent throat irritation.
• If a dose is missed, take it the next morning unless the next scheduled dose is within 7 days. Do not take two tablets in the same week.

Supplemental calcium or vitamin D is recommended if dietary intake is insufficient.

Interactions:

• Calcium supplements, antacids, and medications containing multivalent cations (e.g., aluminum, magnesium, iron) may interfere with Bondrova absorption. Wait 60 minutes after taking Bondrova before consuming these products.
• No significant interactions have been observed with tamoxifen, hormone replacement therapy, or melphalan/prednisolone.

Contraindications:

• Hypersensitivity to Ibandronic Acid or any excipients.
• Uncorrected hypocalcemia.
• Esophageal abnormalities that delay emptying (e.g., stricture, achalasia).
• Inability to sit or stand upright for at least 60 minutes.

Side Effects:
Common side effects include dyspepsia, nausea, diarrhea, abdominal pain, muscle aches, headaches, and dizziness.

Pregnancy and Lactation:

• Avoid using Bondrova during pregnancy or breastfeeding. Studies in animals showed no direct fetal toxicity, but caution is advised due to limited human data.

Precautions and Warnings:

• Correct hypocalcemia and other mineral imbalances before starting treatment.
• Use caution in patients with active gastrointestinal issues (e.g., ulcers, gastritis).
• Monitor for esophageal reactions and discontinue use if symptoms like dysphagia or severe heartburn occur.
• Be aware of the rare risk of osteonecrosis of the jaw (ONJ), especially in patients undergoing dental procedures.

Use in Special Populations:

• Renal Impairment: No dosage adjustment is needed for mild to moderate impairment (creatinine clearance ≥30 ml/min). Use caution in severe cases.
• Hepatic Impairment: No dosage adjustment required.
• Elderly: No dosage adjustment necessary.
• Children: Not recommended for individuals under 18 years.

Overdose:
In case of overdose, provide milk or antacids to bind the medication. Do not induce vomiting, and keep the patient upright to avoid esophageal irritation.

Therapeutic Class:
Bisphosphonate preparations.
