Indications

Brezofil is used to treat the following conditions:

  • Bronchial asthma
  • Bronchospasm
  • Chronic obstructive pulmonary disease (COPD)
  • Pulmonary diseases with a spastic bronchial component

Pharmacology

Doxophylline is a novel bronchodilator, structurally distinct from Theophylline due to the presence of a dioxolane group at position 7. It selectively inhibits phosphodiesterase 4, which helps to relax bronchial smooth muscle. Unlike Theophylline, Doxophylline exhibits reduced affinity for adenosine A1 and A2 receptors, which may explain its improved safety profile. Additionally, Doxophylline is known to inhibit platelet-activating factor (PAF) and decrease the production of leukotrienes, contributing to its therapeutic effects.

Dosage & Administration

Elderly Patients:

  • One 200 mg tablet two or three times daily.

Adults:

  • One 400 mg tablet two or three times daily or as prescribed by the physician.

Children:

  • >12 years of age: One 10 ml syrup or 200 mg tablet two or three times daily.
  • 6-12 years of age: Dosage is based on body weight—6-9 mg/kg body weight twice daily. For example, for a 10 kg child, 3 ml (60 mg) should be given two times daily, or as prescribed by the physician.
  • If the daily required dose of Doxophylline is 400 mg, a Doxophylline SR tablet should be taken once daily, or as prescribed by the physician.

Interaction

Brezofil should not be used in combination with other xanthine-based medications. It is recommended to limit the intake of foods and beverages containing caffeine. Caution should be taken when administering Brezofil with ephedrine or other sympathomimetic drugs.

The co-administration of drugs such as erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, and propranolol may reduce the liver metabolism of xanthine derivatives, leading to higher blood levels of the drug. Conversely, drugs like phenytoin, other anticonvulsants, and cigarette smoking can accelerate the clearance of xanthine derivatives, which might require an adjustment in dosage.

Contraindications

Doxophylline is contraindicated in patients who:

  • Have a known hypersensitivity to the drug or other xanthine derivatives
  • Have experienced an acute myocardial infarction
  • Have hypotension
  • Are lactating

Side Effects

Patients taking xanthine derivatives may experience:

  • Nausea, vomiting, epigastric pain
  • Headache, irritability, insomnia
  • Tachycardia, extrasystoles, tachypnea
  • In rare cases, hyperglycemia or albuminuria

In cases of overdose, severe cardiac arrhythmias and tonic-clonic seizures may occur. These symptoms may indicate intoxication, and the treatment may need to be discontinued. If necessary, at the physician’s discretion, treatment may resume at lower doses after the symptoms of toxicity have resolved.

Pregnancy & Lactation

Pregnancy:
Animal studies have shown that Doxophylline does not interfere with prenatal and postnatal growth. However, since there is insufficient clinical evidence regarding the drug’s effects during pregnancy, its use should be evaluated carefully on a case-by-case basis based on the risk-benefit ratio.

Lactation:
Doxophylline is contraindicated during lactation as there is insufficient data regarding its effects on breastfed infants.

Effects on Ability to Drive and Operate Machinery

Brezofil does not affect the patient’s alertness and therefore does not interfere with their ability to drive or operate machinery.

Precautions & Warnings

Several factors may affect the hepatic clearance of xanthine derivatives, resulting in elevated plasma drug levels. These include:

  • Age
  • Congestive heart failure
  • Chronic obstructive pulmonary disease
  • Severe liver disease
  • Concurrent infections
  • Co-administration of drugs like erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, and propranolol

In such cases, it may be necessary to adjust the dosage and monitor blood drug levels to ensure therapeutic efficacy.

Caution should be observed when prescribing Brezofil to patients with:

  • Cardiac disease
  • Hypertension
  • Elderly patients
  • Severe hypoxemia
  • Hyperthyroidism
  • Chronic corpulmonale
  • Congestive heart failure
  • Liver disease
  • Peptic ulcers
  • Renal impairment

In particular, congestive heart failure requires special caution, as the drug’s clearance may be significantly slower in these patients, leading to prolonged high blood levels even after discontinuation of the drug.

There is no risk of addiction or any form of dependence with Brezofil.

Overdose Effects

In cases of overdose, severe cardiac arrhythmias and tonic-clonic seizures may occur. These are early signs of intoxication. If such symptoms arise, the treatment should be discontinued. After the symptoms subside, the physician may consider resuming the treatment at lower doses. Since there is no specific antidote for overdose, symptomatic treatment of cardiovascular collapse should be initiated.
