Description

Indications
Brodil is a bronchodilator indicated for the treatment of the following conditions:

• Asthma
• Chronic Bronchitis
• Emphysema
• Other respiratory conditions associated with airway obstruction

Pharmacology
Brodil contains Salbutamol, a synthetic sympathomimetic drug that predominantly targets beta-2 adrenergic receptors. By stimulating these receptors in the bronchial smooth muscles, Salbutamol induces bronchodilation, leading to the relaxation of bronchial muscle fibers. This action improves lung function, as shown by spirometric measurements, helping to relieve symptoms of airway obstruction.

Dosage & Administration

• Salbutamol Tablet or Syrup:
  • Children:
    • 2-6 years: 2.5 ml syrup, 3-4 times daily
    • 6-12 years: 5 ml syrup, 3-4 times daily
    • Over 12 years: 5-10 ml syrup, 3-4 times daily (or 2-4 mg tablet, 3-4 times daily)
  • Adults:
    • 2-4 mg tablet, 3-4 times daily. The maximum single dose is 8 mg.
• Salbutamol Respirator Solution:
  This solution is to be used with a nebulizer device under a doctor’s guidance. It must not be injected or ingested.

  Method 1 (Intermittent Administration):

  • Adults & Elderly:
    • 0.5-1.0 ml solution, up to four times a day. A maximum of 40 mg per day can be given under medical supervision in a hospital setting.
    • Dilute 0.5-1.0 ml solution with 2-4 ml of sterile normal saline. The treatment typically takes about 10 minutes.
    • Alternatively, the solution can be used undiluted in a nebulizer for 3-5 minutes until bronchodilation is achieved.
  • Children under 12 years:
    • 0.5 ml solution diluted to 2-4 ml with normal saline, up to four times a day. Some children may require higher doses up to 1.0 ml.
      Method 2 (Continuous Administration):
    • Dilute 1-2 ml solution with 100 ml normal saline. Administer with a nebulizer device. If there is a risk of hypoventilation, oxygen should be added to the inhaled air.
• Salbutamol Nebulizer Solution:
  • Adults & Elderly:
    • 2.5 mg to 5 mg up to 4 times a day. The maximum daily dose is 40 mg under strict medical supervision.
  • Children under 12 years:
    • 2.5 mg, up to 4 times daily. A higher dose of up to 5 mg may be used if required.
• Salbutamol Injection:
  • Adults:
    • Prepare a continuous intravenous infusion by diluting 5 mL of Salbutamol injection solution (1000 mcg/mL) in 500 mL of IV solution to create a 10 mcg/mL concentration.
    • The recommended infusion rate is 3-20 micrograms/minute (0.3-2 mL/minute). Rates may be increased gradually.
    • Inspect the infusion for clarity and particles before use. Discard any unused solution after 24 hours.
  • Children & Adolescents (<18 years):
    • There is insufficient data to recommend a dosage regimen for this age group.
• Salbutamol Inhalation Capsule:
  • Adults:
    • For bronchospasm relief, inhale one or two 200 microgram capsules every 4-6 hours. For maintenance therapy, use one 200 microgram capsule every 4-6 hours. Some patients may need two capsules per dose.
  • Children (4 years and older):
    • One inhalation capsule may be used for acute bronchospasm or for maintenance therapy. For routine use, one capsule may be administered 3-4 times daily, with the possibility of increasing to two capsules if needed.
• Exercise-induced Asthma:
  • Adults: 400 micrograms, 15-30 minutes before exercise.
  • Children: 200 micrograms, 15-30 minutes before exercise.

Side Effects
Common side effects of Brodil may include fine tremors of the skeletal muscles, particularly in the hands, palpitations, muscle cramps, tachycardia (rapid heart rate), headaches, and peripheral vasodilation. In rare cases, higher doses may cause increased tension and adverse cardiovascular effects.

Pregnancy & Lactation
Brodil should be used during pregnancy only if the potential benefits justify the potential risks to the fetus. It is not known whether Salbutamol is excreted in human milk. Due to concerns about tumorigenicity in animal studies, the decision to discontinue breastfeeding or the drug should be made after careful consideration of the mother’s needs.

Precautions & Warnings
Brodil should be used with caution in patients with:

• Hyperthyroidism
• Cardiovascular diseases
• Vascular occlusion disorders
• Hypertension
• Aneurysms

High doses of Brodil may lead to hypokalemia, which could increase the risk of cardiac arrhythmias, especially in patients using digitalis. Tachyphylaxis (decreased effectiveness) may occur with prolonged high doses. Careful monitoring is necessary for patients with diabetes mellitus, closed-angle glaucoma, or those on antihypertensive therapy.

Overdose Effects
Symptoms of overdose may include angina, headaches, nausea, vomiting, and tremors. The preferred treatment for an overdose is the administration of a cardio-selective beta-blocker. However, beta-blockers should be used cautiously in patients with a history of bronchospasm.
