Indications
Brodil is a bronchodilator indicated for the treatment of the following conditions:

• Asthma
• Chronic Bronchitis
• Emphysema
• Other respiratory conditions associated with airway obstruction

Pharmacology
Brodil contains Salbutamol, a synthetic sympathomimetic drug with a primary effect on beta-2 adrenergic receptors. By stimulating these receptors in the bronchial smooth muscles, Salbutamol promotes muscle relaxation, resulting in bronchodilation. This action helps to improve pulmonary function, as demonstrated by spirometric tests.

Dosage & Administration

• Salbutamol Tablet or Syrup

  • Children:
    • 2-6 years: 2.5 ml syrup, 3-4 times daily
    • 6-12 years: 5 ml syrup, 3-4 times daily
    • Over 12 years: 5-10 ml syrup, 3-4 times daily (or 2-4 mg tablet, 3-4 times daily)
  • Adults: 2-4 mg tablet, 3-4 times daily. The maximum single dose is 8 mg.

• Salbutamol Respiratory Solution:
  To be used with a nebulizer device under the guidance of a physician.

  • Intermittent Administration:
    • Adults (including elderly): 0.5-1.0 ml solution, up to four times daily. The total daily dosage may reach 40 mg under strict medical supervision in a hospital setting.
    • Children under 12 years: 0.5 ml of solution diluted to 2-4 ml with saline, up to four times daily. Some children may need higher doses (up to 1.0 ml).
  • Continuous Administration: 1-2 ml of solution diluted to 100 ml with saline. The diluted solution is administered through a nebulizer device. Oxygen should be added to the inspired air when there is a risk of hypoventilation.

• Salbutamol Inhalation Capsule

  • Adults: For bronchodilation and managing asthma, use one or two 200 mcg capsules as a single dose. For maintenance therapy, the recommended dose is one 200 mcg capsule every 4 to 6 hours. In some cases, two capsules may be needed.
  • Children (4 years and older): For acute bronchospasm, one inhalation capsule should be used. For routine maintenance, one capsule can be inhaled 3-4 times a day.
  • Exercise-Induced Asthma:
    • Adults: 400 mcg, 15-30 minutes before physical activity.
    • Children: 200 mcg, 15-30 minutes before physical activity.

Side Effects
Common side effects of Brodil may include:

• Fine tremors of skeletal muscles (especially in the hands)
• Palpitations and muscle cramps
• Tachycardia, nervousness, headaches, and peripheral vasodilatation may occur with high doses.

Pregnancy & Lactation

• Pregnancy: Brodil should only be used during pregnancy if the potential benefits justify the potential risks to the fetus.
• Lactation: It is not known whether Salbutamol is excreted in human milk. Due to potential risks highlighted in animal studies, the use of Brodil during breastfeeding should be carefully evaluated, weighing the importance of the drug for the mother.

Precautions & Warnings

• Caution is advised when using Brodil in patients with:
  • Hyperthyroidism, cardiovascular disease, hypertension, occlusive vascular disorders, and aneurysms.
  • Diabetes mellitus or glaucoma, and those receiving antihypertensive treatment.
  • High doses of Brodil may lead to hypokalemia, increasing the risk of digitalis-induced cardiac arrhythmias.
  • Tachyphylaxis may develop with prolonged use at high doses.
  • Careful monitoring is essential in elderly patients and those with severe conditions such as hypoxemia or heart failure.

Overdose Effects
Signs of overdose may include:

• Angina, headache, nausea, tremors, and vomiting.
  The preferred treatment for overdose is a cardio-selective beta-blocker, but these should be used cautiously, especially in patients with bronchospasm.

Therapeutic Class
Short-acting selective β2-adrenoceptor stimulants.
