Cabolin is primarily used to stop breast milk production (lactation) following childbirth, stillbirth, abortion, or miscarriage. It is also recommended for individuals who decide not to continue breastfeeding. Additionally, Cabolin can be prescribed for treating conditions caused by hormonal imbalances, particularly in cases where there is excessive production of prolactin (hyperprolactinemia).

Description:

Cabolin belongs to the class of dopamine receptor agonists. It works by inhibiting the release of prolactin, a hormone secreted by the pituitary gland. Prolactin is responsible for initiating and maintaining milk production. By decreasing the levels of prolactin, Cabolin helps in treating conditions like hyperprolactinemia (excess prolactin in the bloodstream) and preventing or stopping lactation.


Pharmacology:

Cabergoline, the active ingredient in Cabolin, is a potent dopamine receptor agonist. It acts on the pituitary gland, where it reduces the secretion of prolactin, a hormone mainly regulated by dopamine release from hypothalamic neurons. Cabergoline has a long duration of action and shows a high affinity for D2 dopamine receptors. In vitro studies suggest that Cabergoline directly inhibits prolactin secretion from rat pituitary lactotrophs. Cabergoline’s binding studies show minimal affinity for other receptors like dopamine D1, α1 and α2 adrenergic, and 5HT1, 5HT2 serotonin receptors.


Dosage & Administration:

  • For stopping lactation after childbirth: A single dose of 1 mg on the first day after delivery is recommended.

  • To halt lactation once breastfeeding has begun: Administer 0.25 mg every 12 hours for two days.

  • For reducing prolactin levels in other hormonal imbalances: Start with 0.5 mg Cabergoline (divided into two doses within a week—e.g., half a tablet on Monday and the other half on Thursday). The dose may be gradually increased up to a maximum of 4.5 mg based on the patient’s response.

The initial recommended dosage for starting therapy is 0.25 mg twice a week. Depending on the patient’s serum prolactin levels, the dosage may be increased by 0.25 mg twice weekly, up to a maximum of 1 mg twice a week. Increments should be spaced at least four weeks apart to assess the patient’s response to treatment. If no further benefit is observed with higher doses, it is recommended to use the lowest effective dose that achieves the maximal response. If prolactin levels stabilize for six months, treatment with Cabergoline may be discontinued, with periodic monitoring of prolactin levels.

Interactions:

  • Blood pressure-lowering medications: Caution is advised when using Cabolin with other drugs that reduce blood pressure.
  • Antipsychotic medications: Such as chlorpromazine and haloperidol, may interact with Cabolin.
