Pharmacology:
Nutrition plays a vital role in the prevention of osteoporosis and other bone-related disorders. Calcium, Magnesium, and Vitamin D3 are essential macronutrients for bone health. Vitamin D3 is crucial as it enhances calcium absorption. Magnesium, like calcium, strengthens bones and supports their structural integrity.
Recent studies have also highlighted the importance of Copper, Manganese, Zinc, and Boron as essential micronutrients in maintaining bone density and preventing conditions like osteoporosis. Deficiencies in these micronutrients are often observed in patients with bone health issues.
Dosage & Administration:
-
Adults: Take 1 film-coated tablet twice daily (preferably one in the morning and one in the evening), or as prescribed by your healthcare provider. It is recommended to take the tablets with meals to enhance absorption.
-
Children (ages 3-7): Take 1 effervescent tablet daily.
-
Children (7 years and older): Take 1-2 effervescent tablets daily.
Take the medicine according to the advice of a registered healthcare provider.
Interactions:
- Thiazide diuretics can reduce calcium excretion in urine, increasing the risk of hypercalcemia.
- Calcium and Vitamin D3 may interfere with the effectiveness of phenytoin, barbiturates, and glucocorticoids.
- Foods containing oxalic acid, phosphates, or phytinic acid may reduce calcium absorption.
- Cardiac glycosides (like digoxin) may have altered effects if combined with calcium and Vitamin D3 supplements. Monitoring of ECG and calcium levels is necessary.
- Calcium salts may interfere with the absorption of thyroxine, bisphosphonates, quinolone antibiotics, tetracyclines, and iron. It’s advisable to allow at least 4 hours between taking calcium and these substances.
Contraindications:
Calboplex should not be used in the following conditions:
- Hypercalcemia (elevated calcium levels), especially in conditions like myeloma, bone metastasis, sarcoidosis, or primary hyperparathyroidism.
- Severe renal failure or significant kidney issues.
- Vitamin D3 overdose or hypercalciuria (excess calcium in urine).
- Hypersensitivity to any of the ingredients.
Relative contraindications:
- Osteoporosis due to prolonged immobilization, renal stones, or severe hypercalciuria (excessive calcium in urine).
Side Effects:
Calboplex is generally well tolerated; however, some users may experience:
- Mild gastrointestinal discomfort, such as constipation, flatulence, nausea, gastric pain, or diarrhea.
- Skin rash may occasionally occur after Vitamin D3 supplementation.
- Hypercalciuria (excess calcium in urine) and, in rare cases, hypercalcemia (elevated calcium levels) may occur, especially with long-term or high-dose treatments.
Pregnancy & Lactation:
The use of Calboplex during pregnancy and lactation should be under the supervision of a healthcare professional. Requirements for calcium and Vitamin D3 increase during pregnancy and breastfeeding. Any supplementation should be adjusted based on the individual’s needs and other sources of these nutrients.
Caution: Overdosing on Vitamin D3 has been associated with teratogenic effects in animal studies. Long-term hypercalcemia in humans may result in adverse effects such as developmental delays, aortic stenosis, or retinopathy in newborns.
Both Vitamin D3 and its metabolites can pass into breast milk.
Precautions & Warnings:
- Careful monitoring is required for patients with mild to moderate renal failure or hypercalciuria.
- Regular checks of plasma calcium levels and urinary calcium excretion should be made for these patients.
- Urinary calcium excretion should also be measured in patients with a history of renal stones to rule out hypercalciuria.
- Serum calcium and kidney function should be regularly monitored during long-term treatment. Treatment should be temporarily reduced or stopped if urinary calcium exceeds 7.5 mmol/24 hours.
- When taking both calcium and iron supplements, they should be taken at different times to avoid absorption interference.
Overdose Effects:
Symptoms of an overdose may include:
- Nausea and vomiting
- Severe drowsiness
- Dry mouth and loss of appetite
- Metallic taste
- Stomach cramps, diarrhea, headache, or constipation
Therapeutic Class:
Specific mineral and vitamin combined preparations for bone health.
Storage Conditions:
Store in a cool, dry place, away from light and heat. Keep out of the reach of children.
