Description
Indications
Calboral-D tablet is indicated for the treatment and prevention of:
- Osteoporosis
- Osteomalacia
- Tetany
- Hypoparathyroidism
- Disorders of osteogenesis
It is also used as a supplement in cases of inadequate calcium intake in childhood diets, rickets, pregnancy, lactation, and elderly patients. Additional indications include pancreatitis and as a phosphate binder in chronic renal failure.

Composition
Each tablet contains:
- Calcium Carbonate USP 1250 mg (Coral source) equivalent to 500 mg of elemental Calcium & Vitamin D USP 200 IU.
Each DX tablet contains:
- Calcium Carbonate USP 1500 mg (Coral source) equivalent to 600 mg of elemental Calcium & Vitamin D USP 400 IU.
Each effervescent tablet contains:
- Calcium Carbonate (Coral source) USP 1500 mg equivalent to 600 mg of elemental Calcium and Vitamin D3 400 IU as Cholecalciferol USP.
Pharmacology

Clinical evidence suggests that Calcium is effective in preventing and treating Osteoporosis and related fractures. Vitamin D aids in the absorption of Calcium from the gastrointestinal tract and stimulates bone formation. Studies indicate that Calcium and Vitamin D work synergistically to promote bone growth and prevent fractures. The Calcium Carbonate in this preparation is derived from coral, which has a chemical structure similar to human bone, ensuring better absorption. Vitamin D3 helps maintain Calcium balance in the body.
Dosage & Administration
- One tablet once or twice daily with plenty of water or as directed by a physician. Taking it on a full stomach ensures better absorption.
- Dissolve one effervescent tablet in a glass of water and drink immediately after stirring.

Interaction
- Thiazide diuretics: Reduce urinary calcium excretion, increasing the risk of hypercalcemia. Serum calcium should be monitored regularly.

- Ion exchange resins (e.g., cholestyramine) & laxatives (e.g., paraffin oil): May reduce Vitamin D absorption.
- Tetracycline preparations: Calcium interferes with absorption; tetracyclines should be taken at least two hours before or four to six hours after Calcium intake.
- Cardiac glycosides (e.g., Digoxin): Hypercalcemia may increase toxicity; ECG and calcium levels should be monitored.
- Bisphosphonates & sodium fluoride: Calcium should be taken at least three hours before these medications to prevent reduced absorption.
- Oxalic acid (in spinach, rhubarb) & phytic acid (in whole cereals): May inhibit Calcium absorption. Calcium products should not be taken within two hours of consuming these foods.
Contraindications
- Hypercalcemia and hyperparathyroidism
- Hypercalciuria and nephrolithiasis
- Hypersensitivity to any components of the preparation
- Severe renal insufficiency
- Concomitant Digoxin therapy (requires careful monitoring of serum Calcium levels)
Side Effects
- Gastrointestinal irritation, constipation

Pregnancy & Lactation
- Daily intake should not exceed 1500 mg Calcium and 600 IU Cholecalciferol during pregnancy.
- High doses of Vitamin D may cause reproductive toxicity.
- Overdoses of Calcium and Vitamin D should be avoided due to potential adverse effects on the fetus.
- At therapeutic doses, Vitamin D is not teratogenic.

- Patients with renal impairment should be cautious, as Vitamin D metabolism may be affected.
- Patients with sarcoidosis should be monitored for increased metabolism of Vitamin D.
- Immobilized osteoporosis patients have a higher risk of hypercalcemia.
- Additional Calcium or Vitamin D intake should be supervised by a physician.
Overdose Effects
Overdose may result in hypervitaminosis D and hypercalcemia. Symptoms include:
- Anorexia, thirst, nausea, vomiting, constipation, abdominal pain
- Muscle weakness, fatigue, mental disturbances
- Polydipsia, polyuria, bone pain, nephrocalcinosis, nephrolithiasis
- Severe cases may lead to cardiac arrhythmias, coma, and death
- Persistent hypercalcemia can cause irreversible renal damage and soft tissue calcification
Treatment of Hypercalcemia
- Discontinue Calcium, Thiazide diuretics, Lithium, Vitamin A, and cardiac glycosides.
- Stomach emptying for unconscious patients.
- Rehydration, with possible use of loop diuretics, bisphosphonates, calcitonin, and corticosteroids.
- Monitor serum electrolytes, renal function, diuresis, ECG, and central venous pressure.
