Description
Indications:
Cardizem Film-Coated Tablets are used for the prevention and treatment of chronic stable (classical) angina pectoris and vasospastic angina pectoris. It can be used both alone or in combination for the management of hypertension. Additionally, Cardizem has shown efficacy in the treatment of myocardial infarction, coronary artery spasm, arrhythmias, Raynaud’s phenomenon, esophageal motility disorders, and migraines.
The sustained-release formulation of Cardizem is specifically indicated for the prophylaxis and treatment of angina pectoris. It is effective for both classical and vasospastic types of angina and is also used in managing essential hypertension. In certain cases, it is utilized for the prevention of selected supraventricular tachyarrhythmias.

Pharmacology:
Cardizem contains Diltiazem Hydrochloride, a calcium channel antagonist that works as a peripheral and coronary vasodilator with mild negative inotropic effects. Diltiazem primarily acts by inhibiting calcium influx, particularly at the sino-atrial (S-A) and atrioventricular (A-V) nodes of the heart. Below are its primary pharmacological actions:

Anti-Anginal Action: Diltiazem directly dilates coronary arteries and arterioles, thereby enhancing oxygen supply to the myocardial tissues. In addition, it reduces the “afterload” of the heart by dilating peripheral vasculature, which lowers systemic pressure and reduces myocardial oxygen demand.

Antiarrhythmic Action: By inhibiting calcium ion influx in cardiac tissues, Diltiazem slows electrophysiological activity at the S-A and A-V nodes without affecting accessory bypass conduction or altering normal atrial action potentials.

Antihypertensive Action: Diltiazem reduces peripheral vascular resistance due to vasodilation, which helps lower blood pressure.
Diltiazem is absorbed rapidly from the gastrointestinal tract with over 90% absorption. It is highly protein-bound, and its peak plasma concentration occurs within 30-60 minutes of administration. The drug is metabolized in the liver and excreted primarily (60%) through bile.
Dosage & Administration:
Film-Coated Tablet:
- Usual Dose: The standard starting dose for Diltiazem Hydrochloride film-coated tablets is 60 mg, taken three times daily. Dosage may be adjusted depending on the patient’s response, and it can be increased up to 180-300 mg/day if required. In some cases, doses as high as 480 mg/day may be used, especially for patients with unstable angina.
- Elderly or Patients with Impaired Liver/Renal Function: The recommended starting dose for these patients is 60 mg twice daily. Regular monitoring of heart rate is advised, and if the heart rate drops below 50 beats per minute, the dose should not be increased.
Sustained-Release Tablet:
- For Mild to Moderate Hypertension: Begin with 90 mg or 120 mg twice daily. The dose may be increased as needed up to 360 mg/day (up to 240 mg/day for elderly patients).
- For Angina: The starting dose is 90 mg or 120 mg twice daily, which can be adjusted as necessary. For elderly patients, a maximum of 240 mg/day is recommended.

Interaction:
Cardizem can be used safely alongside short-acting or long-acting nitrates for prophylactic therapy. However, caution is required when administering Diltiazem with other drugs that affect cardiac contractility and conduction. Close monitoring and careful dosage adjustments are recommended.
Contraindications:
Cardizem should not be used in patients who:
- Are hypersensitive to Diltiazem or any of its components.
- Have sick sinus syndrome, second or third-degree AV block, severe hypertension, acute myocardial infarction, or radiographically documented pulmonary congestion.

Common side effects of Cardizem may include:
- Bradycardia
- Sino-atrial block
- Atrioventricular block
- Hypertension
- Malaise
- Headache
- Hot flushes
- Gastrointestinal disturbances (e.g., nausea, vomiting, gastric discomfort)
- Edema
- Hepatitis
- Depression

Pregnancy & Lactation:
- Pregnancy: There are no well-controlled studies in pregnant women. Cardizem should only be used during pregnancy if the potential benefits outweigh the risks to the fetus.
- Lactation: Diltiazem is excreted into breast milk. Therefore, it is not recommended for breastfeeding mothers unless absolutely necessary. Alternative feeding methods should be considered if treatment is required.
