Indications
CardoPlus tablets are primarily indicated for the treatment of hypertension. Additionally, they are used to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy.

Pharmacology
CardoPlus contains two active ingredients: Losartan and Hydrochlorothiazide.

  • Losartan:
    Losartan is an angiotensin II receptor antagonist. Angiotensin II, a potent vasoconstrictor formed from angiotensin I, plays a significant role in the pathophysiology of hypertension. It stimulates aldosterone secretion by the adrenal cortex, leading to sodium retention and an increase in blood pressure. Losartan blocks the effects of angiotensin II by selectively binding to the AT1 receptor in various tissues such as vascular smooth muscle and the adrenal gland. This results in vasodilation and reduced secretion of aldosterone, ultimately lowering blood pressure. Losartan does not inhibit ACE (angiotensin-converting enzyme) and does not interfere with bradykinin metabolism or other important cardiovascular receptors.

  • Hydrochlorothiazide:
    Hydrochlorothiazide is a thiazide diuretic that acts by increasing the excretion of sodium and chloride through the kidneys, which leads to a reduction in plasma volume and, consequently, a decrease in blood pressure. The diuretic action of Hydrochlorothiazide can also result in increased plasma renin activity and aldosterone secretion, which may lead to potassium loss. However, the presence of Losartan, an angiotensin II receptor antagonist, helps counteract the potassium loss associated with Hydrochlorothiazide.


Dosage

  • Hypertension:
    The usual starting dose of CardoPlus (Losartan 50 mg/Hydrochlorothiazide 12.5 mg) is one tablet daily. If necessary, the dosage may be increased to 100/25 mg daily.
    For patients whose blood pressure is not adequately controlled with Losartan 100 mg monotherapy, this combination tablet can be used at 100/12.5 mg daily.
    In hypertensive patients with left ventricular hypertrophy, the initial dose is 50/12.5 mg, which may be increased to 100/12.5 mg and then to 100/25 mg if further blood pressure reduction is required. The maximum dose is 100/25 mg daily.

  • Severe Hypertension:
    The initial dose for severe hypertension is one tablet of 50/12.5 mg daily. If the response is inadequate after 2 to 4 weeks, the dose can be increased to 100/25 mg once daily.

  • Elderly Patients:
    No adjustment of the starting dose (50/12.5 mg) is required for elderly patients. However, the maximum dose of 100/25 mg should not be used as initial therapy in elderly patients.


Administration
CardoPlus can be taken with or without food and may be combined with other antihypertensive treatments. It is recommended to follow the dosage and frequency prescribed by a healthcare professional.

Interactions

  • Losartan:

    • No significant interactions have been observed with Hydrochlorothiazide, Digoxin, Warfarin, Cimetidine, and Phenobarbital.
    • Potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium can increase serum potassium levels when used alongside Losartan.
    • The antihypertensive effect of Losartan may be reduced by NSAIDs, such as Indomethacin.
  • Hydrochlorothiazide:

    • Alcohol, barbiturates, or narcotics may potentiate orthostatic hypotension when used with Hydrochlorothiazide.
    • Antidiabetic drugs may require dosage adjustments when used concurrently with Hydrochlorothiazide.
    • The absorption of Hydrochlorothiazide may be impaired by anionic exchange resins like Cholestyramine and Colestipol.

Contraindications
CardoPlus is contraindicated in patients who are hypersensitive to any component of the drug. It is also contraindicated in individuals with anuria or hypersensitivity to sulfonamide-derived drugs.


Side Effects
Common side effects include dizziness, hypotension, hyperkalemia, angioedema, and gastro-intestinal disturbances. Rare side effects may include liver issues such as hepatitis, syncope, cerebrovascular accidents, and severe allergic reactions like anaphylaxis. Patients may also experience headaches, sleep disorders, urticaria, rash, and more. Please consult a healthcare provider if any side effects occur.


Pregnancy & Lactation
Angiotensin-II receptor antagonists like Losartan should be avoided during pregnancy unless deemed essential by a healthcare provider due to potential risks to the fetus, including adverse effects on blood pressure control and renal function. These drugs have also been linked to skull defects and oligohydramnios in pregnancy.
CardoPlus is not recommended for breastfeeding as Losartan is distributed into breast milk. Alternative treatments should be considered during lactation.


Precautions & Warnings

  • Monitor for potential electrolyte imbalances (e.g., hypokalemia).
  • Regular assessment of renal function is essential for patients with impaired kidney function.
  • Use caution in patients with hepatic impairment and avoid titrating the dosage in these cases.
  • CardoPlus should not be used in patients with severe renal impairment or in pediatric patients, as safety and efficacy have not been established for this group.

Overdose Effects

  • Losartan Potassium: Overdose symptoms may include hypotension and tachycardia, with potential bradycardia from vagal stimulation. If symptomatic hypotension occurs, supportive treatment should be initiated.
  • Hydrochlorothiazide: Overdose can lead to electrolyte depletion, including hypokalemia and dehydration, especially if combined with digitalis. Hemodialysis is not effective in removing Hydrochlorothiazide.
