Indications
Carva is used for the following conditions:

  • Prevention of Arterial Occlusive Events: This includes conditions such as myocardial infarction (heart attack), recurrent myocardial infarction, post-bypass surgery care, and the prevention of acute ischemic stroke or transient ischemic attack (TIA).

  • Mild to Moderate Pain: Carva is effective for managing various types of pain, including headaches, muscle pain, dysmenorrhea (painful menstruation), and toothaches.

  • Chronic Pain and Inflammation: It is indicated for conditions like osteoarthritis, where inflammation and pain persist over time.

  • Antipyretic (Fever Reduction): Carva is useful in reducing fever associated with cold and influenza.

Pharmacology

Carva contains Aspirin, which works by inhibiting platelet aggregation, thereby preventing thrombus (blood clot) formation in arteries. Since anticoagulants have little effect on arterial thrombi, Aspirin is especially effective in this context.

Aspirin is recognized for its:

  • Analgesic (pain-relieving) properties, making it the preferred treatment for headaches, musculoskeletal pain, and dysmenorrhea.
  • Anti-inflammatory effects, which help reduce swelling and discomfort associated with chronic inflammatory conditions like osteoarthritis.
  • Antipyretic (fever-reducing) effects, which help in managing fevers caused by illnesses like the flu.

The enteric coating on Aspirin helps minimize gastrointestinal disturbances and the risk of ulcers that can occur with regular Aspirin use.


Dosage & Administration

  • For Pain, Inflammatory Conditions, and Fever: Take 300 mg (1-3 tablets) every 6 hours, with a maximum daily dose of 4 gm.

  • For Suspected Acute Coronary Syndrome: Administer 150 mg to 300 mg immediately, unless there are contraindications.

  • After Myocardial Infarction: Take 150 mg daily for the first month. Thereafter, reduce the dose to 75 mg daily for long-term use.

  • For Acute Ischemic Stroke/Transient Ischemic Attack (TIA): Start with 150 mg to 300 mg daily, followed by 75 mg daily thereafter.

  • Post-Surgery (Bypass Surgery): Start with 75 mg to 300 mg daily, beginning 6 hours after the procedure.

Drug Interactions

Carva may interact with several medications, potentially altering their effectiveness or increasing the risk of side effects. Key interactions include:

  • Anticoagulants, oral hypoglycemic agents, phenytoin, and sodium valproate: These medications may have enhanced effects when taken with Carva.
  • Probenecid: Carva may reduce the uricosuric effect of probenecid.
  • Sulphonamides: The toxicity of sulphonamides may be increased when taken alongside Carva.
  • Asthma: People with asthma may experience bronchospasm or exacerbation of asthma symptoms.

Contraindications

Carva should not be used by individuals who:

  • Are under 12 years of age (due to the risk of Reye’s syndrome).

  • Have intracranial hemorrhage or other forms of bleeding.

Side Effects

Carva is generally well-tolerated, but some individuals may experience mild side effects such as:

Precautions & Warnings

  • Carva should be used with caution in patients with:

    • Nasal polyp and nasal allergies.
  • Be mindful of possible liver function abnormalities, especially in patients with underlying health conditions such as AIDS or cancer.

  • If a rash develops during treatment, especially with systemic fungal infections, discontinue use and consult a doctor.


Overdose Effects

Overdose symptoms may include:

  • Dizziness, tinnitus (ringing in the ears), sweating, nausea, and vomiting.
  • Confusion, hyperventilation, and in severe cases, CNS depression, coma, cardiovascular collapse, and respiratory depression.

In the event of an overdose, immediate medical attention is required. Treatment may include:

  • Gastric lavage and forced alkaline diuresis.
  • Hemodialysis may be necessary in severe cases.
