Indications
Cef-3 is an orally active cephalosporin antibiotic with significant in-vitro bactericidal activity against a wide range of Gram-positive and Gram-negative organisms. It is indicated for treating the following acute infections caused by susceptible microorganisms:

  • Upper Respiratory Tract Infections (URTI): Including otitis media and other URTIs where the causative organism is known or suspected to be resistant to other commonly used antibiotics or where treatment failure may pose significant risks.
  • Lower Respiratory Tract Infections: Including bronchitis.
  • Urinary Tract Infections: Including cystitis, cystourethritis, and pyelonephritis. Cef-3 has demonstrated efficacy against common pathogens like Streptococcus pneumoniae, Streptococcus pyogenes, Escherichia coli, Proteus mirabilis, Klebsiella species, Haemophilus influenzae (both beta-lactamase positive and negative), Moraxella catarrhalis (both beta-lactamase positive and negative), and Enterobacter species. Cef-3 is highly stable in the presence of beta-lactamase enzymes.

Pharmacology
Cefixime is a third-generation semisynthetic cephalosporin antibiotic for oral administration. It is bactericidal against a broad spectrum of Gram-positive and Gram-negative bacteria at easily achievable plasma concentrations. It works by interfering with bacterial cell wall synthesis. Cefixime is highly stable in the presence of beta-lactamase enzymes, making it effective against organisms resistant to penicillins and some cephalosporins. Absorption is approximately 40-50% whether administered with or without food.

Dosage & Administration

  • Adults and Children Over 10 Years: The recommended dosage is 200-400 mg daily, depending on the severity of the infection, given either as a single dose or in two divided doses.
  • Elderly: Elderly patients may receive the same dose as adults, with renal function assessed for adjustments in cases of severe renal impairment.
  • Children: The recommended dose for children is 8 mg/kg/day, administered as a single dose or in two divided doses. The following daily doses in terms of volume of suspension are suggested:
    • 6 months to 1 year: 3.75 ml
    • 1-4 years: 5 ml
    • 5-10 years: 10 ml
    • For typhoid, the recommended dose is 5 mg/kg body weight twice daily for 10-14 days.
  • Renal Impairment: Normal dosing is recommended for patients with creatinine clearances of 20 ml/min or greater. For patients with creatinine clearance less than 20 ml/min, a maximum daily dose of 200 mg is advised.

Interaction
Cefixime may increase prothrombin times in some patients, so caution is advised when used with anticoagulant therapy.

Contraindications
Patients with known hypersensitivity to cephalosporin antibiotics should not use Cef-3.

Side Effects
Cef-3 is generally well tolerated, with most adverse reactions being mild and self-limiting. Common side effects include:

  • Gastrointestinal disturbances: Diarrhea (more common with higher doses), nausea, abdominal pain, dyspepsia, vomiting, flatulence, and pseudomembranous colitis.
  • Central Nervous System: Headache and dizziness.
  • Hypersensitivity reactions: Rash, pruritus, urticaria, drug fever, and arthralgia.
  • Hematological and Chemistry: Thrombocytopenia, leukopenia, and eosinophilia (infrequent and reversible).
  • Miscellaneous: Genital pruritus and vaginitis.

Pregnancy & Lactation
There are no adequate and well-controlled studies in pregnant women. Cef-3 should only be used during pregnancy if clearly needed. It is not known if Cefixime is excreted in human milk, so caution is advised when administering it to nursing women.

Precautions & Warnings

  • Cef-3 should be used cautiously in patients with a history of hypersensitivity to other drugs, particularly penicillin, as there is a risk of cross-reactivity between penicillins and cephalosporins.
  • Caution is advised in patients with impaired renal function.
  • Treatment with broad-spectrum antibiotics may alter the normal flora of the colon, allowing overgrowth of Clostridium difficile, leading to antibiotic-associated diarrhea.

Overdose Effects
In case of overdose, gastric lavage may be indicated. Cef-3 is not significantly removed from the circulation by hemodialysis or peritoneal dialysis.
