Indications

Cefaclav is a combination antibiotic indicated for the treatment of various infections caused by susceptible bacteria. It is effective in managing the following conditions:

  • Pharyngitis/Tonsillitis: Caused by Streptococcus pyogenes.
  • Acute Bacterial Otitis Media: Caused by Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis (including beta-lactamase-producing strains), or Streptococcus pyogenes.
  • Acute Bacterial Maxillary Sinusitis: Caused by Streptococcus pneumoniae or Haemophilus influenzae (non-beta-lactamase producing strains).
  • Lower Respiratory Tract Infections: Including pneumonia, caused by Streptococcus pneumoniae, Haemophilus influenzae (including beta-lactamase producing strains), Klebsiella spp., Staphylococcus aureus (both penicillinase- and non-penicillinase-producing strains), Streptococcus pyogenes, and E. coli.
  • Acute Bacterial Exacerbation of Chronic Bronchitis: Caused by Streptococcus pneumoniae, Haemophilus influenzae (non-beta-lactamase strains), or Haemophilus parainfluenzae (non-beta-lactamase strains).
  • Uncomplicated Skin and Skin Structure Infections: Caused by Staphylococcus aureus (including beta-lactamase-producing strains) or Streptococcus pyogenes.
  • Uncomplicated Urinary Tract Infections: Caused by E. coli or Klebsiella pneumoniae.
  • Bone and Joint Infections: Caused by Staphylococcus aureus (both penicillinase- and non-penicillinase-producing strains).
  • Uncomplicated Gonorrhea: Caused by both penicillinase-producing and non-penicillinase-producing strains of Neisseria gonorrhoeae.
  • Early Lyme Disease (Erythema Migrans): Caused by Borrelia burgdorferi.
  • Septicemia: Caused by Staphylococcus aureus, Streptococcus pneumoniae, E. coli, Haemophilus influenzae (including ampicillin-resistant strains), and Klebsiella spp.
  • Meningitis: Caused by Streptococcus pneumoniae, Haemophilus influenzae (including ampicillin-resistant strains), Neisseria meningitidis, and Staphylococcus aureus (both penicillinase- and non-penicillinase-producing strains).

Pharmacology

Cefaclav combines Cefuroxime, a second-generation cephalosporin antibiotic, with Clavulanic acid, a beta-lactamase inhibitor.

  • Cefuroxime is a bactericidal agent that targets bacterial cell wall synthesis by inhibiting transpeptidation, effective against a broad spectrum of Gram-positive and Gram-negative organisms, including many beta-lactamase-producing strains.
  • Clavulanic acid, produced by Streptomyces clavuligerus, inhibits beta-lactamase enzymes. It prevents the degradation of Cefuroxime, thereby enabling effective treatment against beta-lactam-resistant bacteria.

Dosage and Administration

For Adolescents and Adults (13 years and older):

  • Pharyngitis/Tonsillitis: 250 mg twice daily for 5-10 days.
  • Acute Bacterial Maxillary Sinusitis: 250 mg twice daily for 10 days.
  • Acute Bacterial Exacerbation of Chronic Bronchitis: 250-500 mg twice daily for 10 days.
  • Secondary Bacterial Infections of Acute Bronchitis: 250-500 mg twice daily for 5-10 days.
  • Uncomplicated Skin and Skin Structure Infections: 250-500 mg twice daily for 10 days.
  • Uncomplicated Urinary Tract Infections: 250 mg twice daily for 7-10 days.
  • Uncomplicated Gonorrhea: 1000 mg twice daily, single dose.
  • Community Acquired Pneumonia: 250-500 mg twice daily for 5-10 days.
  • MDR Typhoid Fever: 500 mg twice daily for 10-14 days.
  • Early Lyme Disease (Erythema Migrans): 500 mg twice daily for 20 days.

For Paediatric Patients (3 months to 12 years):

  • Pharyngitis/Tonsillitis: 20 mg/kg/day twice daily for 5-10 days.
  • Acute Otitis Media: 30 mg/kg/day twice daily for 10 days.
  • Acute Bacterial Maxillary Sinusitis: 30 mg/kg/day twice daily for 10 days.
  • Impetigo: 30 mg/kg/day twice daily for 10 days.

Cefuroxime-Clavulanic Acid tablets can be taken regardless of food intake.

Interactions

When taken alongside probenecid, the area under the serum concentration-time curve of Cefuroxime increases by 50%. Medications that reduce gastric acidity may lower the bioavailability of Cefuroxime, diminishing the effects of postprandial absorption.

Contraindications

Cefuroxime-Clavulanic Acid should not be used by patients with a known allergy to cephalosporins or those suffering from Pseudomembranous Colitis.

Side Effects

Cefuroxime-Clavulanic Acid is generally well tolerated. However, some side effects may occur, including nausea, vomiting, diarrhea, and abdominal discomfort. Prolonged use can lead to overgrowth of non-susceptible microorganisms. Rare side effects (<0.2%) include renal dysfunction, anaphylaxis, angioedema, pruritus, rash, and serum sickness-like urticaria.

Pregnancy and Lactation

While antibiotics are best avoided during the first trimester of pregnancy, Cefuroxime-Clavulanic Acid is considered safe for use during later stages to treat urinary and other infections. It is excreted into breast milk in small amounts; caution should be taken to monitor for potential sensitization in the infant.

Precautions and Warnings

Cefuroxime should be administered cautiously in patients on concurrent potent diuretics or those with a history of colitis.
