Indications
Cefim-3 is an orally active cephalosporin antibiotic with significant bactericidal activity against a broad range of Gram-positive and Gram-negative organisms. It is indicated for treating acute infections caused by susceptible microorganisms, including:
- Upper Respiratory Tract Infections (URTI): e.g., otitis media and other URTIs resistant to common antibiotics or where treatment failure poses significant risks.
- Lower Respiratory Tract Infections: e.g., bronchitis.
- Urinary Tract Infections: e.g., cystitis, cystourethritis, pyelonephritis.
Clinical efficacy has been shown against pathogens such as Streptococcus pneumonia, Streptococcus pyogenes, Escherichia coli, Proteus mirabilis, Klebsiella species, Haemophilus influenzae (both beta-lactamase positive and negative), Moraxella catarrhalis (both beta-lactamase positive and negative), and Enterobacter species. Cefim-3 is highly stable in the presence of beta-lactamase enzymes.

Pharmacology
Cefim-3 is a third-generation semisynthetic cephalosporin antibiotic that acts bactericidally by inhibiting bacterial cell wall synthesis. It is effective against both Gram-positive and Gram-negative bacteria at plasma concentrations easily achievable in humans. Cefixime remains stable against beta-lactamase enzymes, which allows it to treat infections caused by organisms resistant to penicillins and some cephalosporins. Its absorption is around 40-50%, whether taken with or without food.
Dosage & Administration
- Adults and Children over 10 years: The recommended dose is 200-400 mg per day, either as a single dose or divided into two doses, depending on infection severity.
- Elderly: Same dosage as adults, with adjustments made for severe renal impairment.
- Children:
- 6 months to 1 year: 3.75 ml daily
- 1-4 years: 5 ml daily
- 5-10 years: 10 ml daily
- For typhoid, 5 mg/kg twice daily for 10-14 days.
- Children above 50 kg or over 10 years should receive the adult dose.
- Renal Impairment:
- For creatinine clearance >20 ml/min: Regular dosing.
- For creatinine clearance <20 ml/min: Limit to 200 mg once daily.

Interaction
Cefim-3 can increase prothrombin times in some patients, particularly those on anticoagulant therapy. Exercise caution in such cases.
Contraindications
- Hypersensitivity to cephalosporin antibiotics.
Side Effects
Cefim-3 is generally well tolerated, but side effects may include:
- Gastrointestinal: Diarrhea, nausea, abdominal pain, vomiting, dyspepsia, flatulence.
- CNS: Headache, dizziness.
- Allergic: Rash, pruritus, urticaria, drug fever, arthralgia.
- Hematological: Thrombocytopenia, leukopenia, eosinophilia (rare).
- Other: Mild liver or renal function changes, genital pruritus, vaginitis.
Pregnancy & Lactation
There are no well-controlled studies in pregnant women. Cefim-3 should only be used during pregnancy if clearly needed. The excretion of Cefim-3 in human milk is not known; caution is advised when used during breastfeeding.
Precautions & Warnings
- Use with caution in patients allergic to penicillin, as there may be cross-reactivity between penicillins and cephalosporins.
- Caution in patients with severe renal impairment.
- Prolonged use may lead to overgrowth of Clostridium difficile, resulting in antibiotic-associated diarrhea.
- Not recommended in pregnant or breastfeeding women unless deemed essential by a doctor.
Overdose Effects
No specific antidote. Gastric lavage may be necessary. Cefim-3 is not significantly removed by dialysis. Overdosing on up to 2g of Cefim-3 has not shown significant adverse effects compared to standard dosing.
