Cefim-3 DS 400 mg
Description
Indications
Cefim-3 DS is an oral cephalosporin antibiotic that demonstrates potent bactericidal activity against a broad spectrum of both Gram-positive and Gram-negative bacteria. It is indicated for the treatment of various acute infections caused by susceptible microorganisms, including:
- Upper Respiratory Tract Infections (URTI): For conditions like otitis media and other URTIs, particularly when the pathogen is known or suspected to be resistant to commonly prescribed antibiotics, or when treatment failure could pose significant risks.
- Lower Respiratory Tract Infections: Such as bronchitis.
- Urinary Tract Infections (UTI): Including cystitis, cystourethritis, and pyelonephritis.
Cefim-3 DS has shown clinical efficacy against common pathogens like Streptococcus pneumoniae, Streptococcus pyogenes, Escherichia coli, Proteus mirabilis, Klebsiella species, Haemophilus influenzae (both beta-lactamase positive and negative), Moraxella catarrhalis (both beta-lactamase positive and negative), and Enterobacter species. It is highly stable against beta-lactamase enzymes, making it effective against organisms resistant to penicillins and other cephalosporins.
Take this medication only under the guidance of a registered medical professional.
Pharmacology
Cefixime, the active ingredient in Cefim-3 DS, is a third-generation semisynthetic cephalosporin antibiotic that is bactericidal in action. It works by inhibiting bacterial cell wall synthesis, an essential process for bacterial survival. Cefixime has strong bactericidal activity against both Gram-positive and Gram-negative bacteria, and its stability against beta-lactamase enzymes helps combat resistant organisms. The absorption of Cefixime ranges between 40-50%, whether taken with food or on an empty stomach.
Dosage & Administration
- Adults and children over 10 years: The typical dosage is 200-400 mg daily, adjusted based on the severity of the infection, and can be taken as a single dose or in two divided doses.
- Elderly: Elderly patients may follow the same dosing guidelines as adults. Renal function should be assessed, and dosage adjustments should be made in cases of severe renal impairment.
- Children:
- Children aged 6 months to 1 year: 3.75 ml daily.
- Children aged 1-4 years: 5 ml daily.
- Children aged 5-10 years: 10 ml daily.
- For typhoid in children, the dose is 5 mg/kg body weight twice daily for 10-14 days.
- Children over 10 years or weighing more than 50 kg should be given the adult dosage of 200-400 mg daily, based on infection severity.
- Renal Impairment:
- For patients with creatinine clearance greater than 20 ml/min, the normal dosage can be administered.
- For creatinine clearance less than 20 ml/min, the maximum recommended dose is 200 mg once daily.
- Dialysis patients should follow the same dosage guidelines as those with severe renal impairment.
Take this medication only as prescribed by a registered medical professional.
Interaction
Similar to other cephalosporins, Cefim-3 DS may increase prothrombin time in a small number of patients, so care should be taken when co-administered with anticoagulant medications.
Contraindications
Cefim-3 DS should not be used in patients with known hypersensitivity to cephalosporin antibiotics.
Side Effects
Cefim-3 DS is generally well tolerated. Common side effects include:
- Gastrointestinal: Diarrhea (more common with higher doses), abdominal pain, nausea, dyspepsia, vomiting, and flatulence.
- Central Nervous System: Headache and dizziness.
- Hypersensitivity: Skin rash, itching, urticaria, drug fever, and joint pain (arthralgia). These reactions usually resolve once the medication is discontinued.
- Hematological: Thrombocytopenia, leukopenia, and eosinophilia (rare and reversible).
- Liver and Renal Function: Mild, transient changes in liver and kidney function tests.
- Other: Genital itching and vaginitis.
Pregnancy & Lactation
There are no well-controlled studies in pregnant women. Animal studies may not always predict human response, so this medication should be used during pregnancy only if absolutely necessary. The excretion of Cefixime in breast milk is unknown; therefore, caution is advised when administering it to nursing mothers.
Precautions & Warnings
Cefim-3 DS should be used with caution in patients with known hypersensitivity to other drugs, particularly in those with a history of penicillin allergy. Some evidence suggests partial cross-reactivity between penicillins and cephalosporins.
Use with caution in patients with significantly impaired renal function. Antibiotic treatment may alter the normal gut flora, potentially leading to overgrowth of Clostridium difficile and antibiotic-associated diarrhea.
Overdose Effects
If overdose occurs, gastric lavage may be required. No specific antidote exists, and Cefim-3 DS is not significantly removed from the body via hemodialysis or peritoneal dialysis. Adverse reactions observed in trials with doses up to 2 g were similar to those seen at therapeutic doses.
Therapeutic Class
Third-generation cephalosporins.
Storage Conditions
Store below 30°C, protected from light and moisture. Keep out of reach of children.
