Indications:
Ceftid is an orally active cephalosporin antibiotic with significant in-vitro bactericidal activity against a wide range of Gram-positive and Gram-negative organisms. It is indicated for treating various acute infections caused by susceptible microorganisms:
- Upper Respiratory Tract Infections (URTI): For conditions like otitis media and other URTIs where the causative organism is resistant to commonly used antibiotics or where treatment failure could pose significant risk.
- Lower Respiratory Tract Infections: Such as bronchitis.
- Urinary Tract Infections (UTI): Including cystitis, cystourethritis, and pyelonephritis. Proven efficacy against pathogens such as Streptococcus pneumoniae, Escherichia coli, Proteus mirabilis, Klebsiella species, and Haemophilus influenzae (both beta-lactamase positive and negative).
Pharmacology:
Cefixime is a third-generation semisynthetic cephalosporin antibiotic for oral administration. It is bactericidal against a broad spectrum of both Gram-positive and Gram-negative bacteria at achievable plasma concentrations. It kills bacteria by disrupting the synthesis of the bacterial cell wall and is highly stable against beta-lactamase enzymes. This allows Cefixime to be effective against bacteria resistant to penicillins and some cephalosporins. Its absorption is about 40% to 50%, regardless of food intake.
Dosage & Administration:
- Adults and children over 10 years: 200-400 mg daily, either as a single dose or in two divided doses.
- Elderly: Can take the same dose as adults, but renal function should be assessed, and adjustments may be needed in cases of severe renal impairment.
- Children:
- 6 months to 1 year: 3.75 ml daily.
- 1-4 years: 5 ml daily.
- 5-10 years: 10 ml daily.
- For typhoid: 5 mg/kg body weight twice daily for 10-14 days.
- Children over 50 kg or older than 10 years: Same as adult dosage.
- Renal Impairment:
- Normal dose for creatinine clearance ≥20 ml/min.
- In cases of creatinine clearance <20 ml/min, limit to a maximum of 200 mg once daily.
Interactions:
Cefixime may increase prothrombin times in some patients, particularly when combined with anticoagulant therapy.
Contraindications:
- Known hypersensitivity to cephalosporins.
Side Effects:
Common and mild side effects include:
- Gastrointestinal: Diarrhea, nausea, abdominal pain, and vomiting.
- Central Nervous System: Headache and dizziness.
- Hypersensitivity Reactions: Rash, pruritis, urticaria, and drug fever.
- Hematological: Thrombocytopenia, leukopenia, eosinophilia (rare and reversible).
Pregnancy & Lactation:
- No adequate studies in pregnant women; use only if necessary.
- Cefixime may be excreted in human milk. Exercise caution when administering to breastfeeding women.
Precautions & Warnings:
- Use with caution in patients with known hypersensitivity to other drugs, particularly penicillins.
- Caution in patients with impaired renal function.
- Risk of Clostridium difficile-associated diarrhea due to broad-spectrum antibiotic use.
Overdose Effects:
- No specific antidote; gastric lavage may be considered.
- Cefixime is not significantly removed by hemodialysis or peritoneal dialysis.
Storage Conditions:
- Store below 30ºC, protected from light and moisture. Keep out of reach of children.
