Indications

Ceftid is an orally active cephalosporin antibiotic with significant in-vitro bactericidal activity against a broad range of Gram-positive and Gram-negative organisms. It is indicated for the treatment of the following acute infections caused by susceptible microorganisms:

  • Upper Respiratory Tract Infections (URTI): e.g., otitis media and other URTIs where the causative organism is resistant to other commonly used antibiotics or where treatment failure carries significant risk.
  • Lower Respiratory Tract Infections (LRTI): e.g., bronchitis.
  • Urinary Tract Infections (UTI): e.g., cystitis, cystourethritis, pyelonephritis.

It is effective against common pathogens such as Streptococcus pneumonia, Streptococcus pyogenes, Escherichia coli, Proteus mirabilis, Klebsiella species, Haemophilus influenzae (beta-lactamase positive and negative), Moraxella catarrhalis (beta-lactamase positive and negative), and Enterobacter species. Ceftid is highly stable in the presence of beta-lactamase enzymes.

Pharmacology

Ceftid (Cefixime) is a third-generation semisynthetic cephalosporin antibiotic used for oral administration. It has a bactericidal effect against a wide spectrum of Gram-positive and Gram-negative bacteria by inhibiting the synthesis of the bacterial cell wall.

  • Absorption: Approximately 40-50% is absorbed, whether taken with or without food.
  • Beta-lactamase stability: It remains effective against organisms resistant to penicillins and some cephalosporins due to the presence of beta-lactamases.

Dosage & Administration

  • Adults and children over 10 years: The usual recommended dosage is 200-400 mg daily depending on the severity of the infection, administered as a single dose or divided into two doses.
  • Elderly: The same dosage as adults can be used, but renal function should be assessed, and dosage may need to be adjusted in cases of severe renal impairment.
  • Children:
    • 6 months to 1 year: 3.75 ml daily.
    • 1-4 years: 5 ml daily.
    • 5-10 years: 10 ml daily.
    • Typhoid (children): 5 mg/kg body weight twice daily for 10-14 days.
    • Children over 10 years or weighing more than 50 kg: Treated with the adult dosage (200-400 mg daily).
    • Children under 6 months: Safety and efficacy have not been established in this age group.
  • Renal Impairment:
    • Normal dosing can be used for patients with creatinine clearance of 20 ml/min or greater.
    • For those with creatinine clearance less than 20 ml/min, the recommended dose is 200 mg once daily, with no more than this dose.
    • For patients on chronic ambulatory peritoneal dialysis or hemodialysis, the dosing recommendations for impaired renal function should be followed.

Interaction

  • Anticoagulants: Ceftid may increase prothrombin times in a few patients, especially those on anticoagulant therapy, so caution is advised.

Contraindications

  • Hypersensitivity to cephalosporin antibiotics.

Side Effects

Ceftid is generally well-tolerated, but side effects may include:

  • Gastrointestinal disturbances: Diarrhea (most common, especially at higher doses), nausea, abdominal pain, dyspepsia, vomiting, flatulence, and pseudomembranous colitis.
  • Central nervous system: Headache and dizziness.
  • Hypersensitivity reactions: Rash, pruritus, urticaria, drug fever, and arthralgia (usually resolve upon discontinuation of therapy).
  • Hematological and clinical chemistry: Thrombocytopenia, leukopenia, and eosinophilia (reversible).
  • Liver and renal function: Mild, transient changes in function tests.
  • Miscellaneous: Genital pruritus and vaginitis.

Pregnancy & Lactation

  • There are no adequate and well-controlled studies in pregnant women. The drug should only be used during pregnancy if clearly needed.
  • It is not known whether Ceftid is excreted in human milk. Caution is advised when administered to a nursing woman.

Precautions & Warnings

  • Hypersensitivity: Ceftid should be used with caution in patients who have shown hypersensitivity to other drugs. Cephalosporins may show partial cross-reactivity in patients sensitive to penicillins.
  • Renal function: Caution is needed for patients with markedly impaired renal function.
  • Clostridium difficile-associated diarrhea: As with other broad-spectrum antibiotics, Ceftid may alter the normal flora of the colon, promoting Clostridium difficile overgrowth and resulting in antibiotic-associated diarrhea.

Overdose Effects

  • Gastric lavage may be necessary. There is no specific antidote for overdose.
  • Ceftid is not significantly removed from circulation by hemodialysis or peritoneal dialysis.
  • Adverse reactions were not significantly different in healthy adults who received single doses up to 2 g compared to patients on the recommended doses.
