Indications

Acute Cerebrovascular Accidents (Strokes):
This medication is used for treating ischemic strokes resulting from cerebral thrombosis, cerebral embolism, acute circulatory disorders, hypertensive crises, and other related conditions. It is beneficial in managing acute cardiovascular disorders, ischemic neurological deficits, complete stroke (CS), multi-infarct dementia, cerebral arteriosclerosis, hypertensive encephalopathy, and post-apoplectic conditions, including those caused by hemorrhagic strokes. -
Senile Disorders:
It provides relief for psychosomatic symptoms in the elderly due to cerebral insufficiency, including forgetfulness, memory disturbances, cognitive slowdowns, difficulty concentrating, dizziness, mood instability, aphasia, sleep disturbances, and vasovegetative symptoms related to menopause. -
Visual Disorders:
This medication helps manage vascular disorders of the choroid and retina caused by arteriosclerosis. It is also used to treat conditions such as vasospasm, macular degeneration, arterial or venous thrombosis, embolism, and glaucoma that result from these vascular issues. -
Hearing Disorders:
It is effective in treating hearing impairment caused by vascular or toxic factors (iatrogenic). It is also used for presbycusis, Meniere’s disease, cochleovestibular neuritis, tinnitus, and dizziness originating from the labyrinth.

Pharmacology
Vinpocetine enhances cerebral metabolism by increasing glucose and oxygen consumption. It boosts the brain’s tolerance to hypoxia and shifts glucose metabolism to a more energy-efficient aerobic pathway, while also supporting anaerobic metabolism. Vinpocetine elevates ATP levels and the ATP/AMP ratio in the brain, as well as increasing cerebral levels of norepinephrine, dopamine, and serotonin.
The drug significantly improves cerebral microcirculation by inhibiting platelet aggregation, reducing excessive blood viscosity, and increasing the deformability of erythrocytes. It also improves oxygen transport to tissues by lowering the oxygen affinity of red blood cells.
Vinpocetine selectively enhances cerebral blood flow without affecting systemic circulation (such as blood pressure, heart rate, and cardiac output). It reduces cerebral vascular resistance, focusing blood supply on injured or ischemic areas (inverse steal effect), without affecting healthy regions. This leads to an overall improvement in blood flow, particularly in hypoxic areas.
Dosage & Administration

Tablet: Take 1-2 tablets three times a day. The maintenance dose is typically 1 tablet three times a day.

IM Injection: Administer a daily dose of 20-40 mg until improvement in symptoms is observed, typically within 10 days. Afterward, transition to oral treatment. If no improvement is seen, consider starting infusion therapy.

IV Infusion: The starting dose is 20 mg via slow drip infusion (diluted in 500-1000 ml of infusion solution). The dose can be increased to 1 mg/kg body weight over the course of 3-4 days. Continue treatment for 10-14 days, depending on patient tolerance. Gradually reduce the dose before discontinuing treatment.

Interactions
This medication should not be mixed with heparin in the same syringe due to chemical incompatibility.
Contraindications
Parenteral administration (injection or infusion) is contraindicated in individuals with severe ischemic heart disease, significant rhythm disturbances, or during pregnancy.
Side Effects
Possible side effects include transient hypotension (low blood pressure) and tachycardia (increased heart rate).
Pregnancy & Lactation
This medication is contraindicated during pregnancy and lactation.
Precautions & Warnings
For acute conditions, parenteral treatment is recommended until symptoms improve, followed by oral treatment. For chronic cases, oral therapy should be continued.
