Description

Indications:

Cildip is prescribed for the effective management of hypertension (high blood pressure), aimed at protecting end organs from damage caused by elevated blood pressure. It is particularly beneficial in elderly patients, those with diabetes, and individuals with albuminuria (presence of abnormal amounts of protein in the urine). Cildip is also increasingly utilized in patients suffering from chronic kidney disease to help manage blood pressure and reduce complications.

Hypertension occurs when there is excessive pressure exerted on the walls of the blood vessels due to the blood pumped by the heart. Over time, this elevated pressure can lead to damage of the blood vessels and other organs.

Pharmacology:

Cildip contains Cilnidipine, a dihydropyridine calcium-channel blocker. Cilnidipine works by inhibiting calcium ions (Ca²⁺) from entering the vascular smooth muscle cells through the L-type voltage-dependent calcium channels. This process relaxes the vascular smooth muscles and leads to vasodilation (widening of the blood vessels), which in turn lowers blood pressure.

In addition, Cilnidipine blocks calcium influx via N-type voltage-dependent calcium channels in the sympathetic nerve cell membranes, reducing the release of norepinephrine. By suppressing norepinephrine release, Cilnidipine helps reduce the reflex increase in heart rate that often occurs when blood pressure is lowered. This dual action of Cilnidipine makes it effective in controlling blood pressure without significantly raising the heart rate.


Dosage and Administration:

  • Adults: Take 5-10 mg once daily after breakfast. The maximum dose is 20 mg once daily.

  • Pediatric Use: The safety and efficacy of Cilnidipine in children have not been established, so its use in pediatric patients is not recommended.

  • Elderly Use: Elderly patients may be more sensitive to the effects of Cilnidipine. It is recommended to begin treatment with the lowest possible dose (5 mg) and adjust as needed under medical supervision.

Interaction:

Cildip may interact with other medications, including:

  • Other anti-hypertensives or antipsychotic drugs that cause low blood pressure.
  • Quinidine, carbamazepine, phenytoin, rifampicin, cimetidine, and erythromycin.

Contraindications:

Cildip should not be used by individuals who:

  • Are allergic to Cilnidipine or any of its components.

  • Flushing

  • Palpitations (irregular heartbeats)

  • Increased frequency of urination
  • Lethargy (fatigue)
  • Eye pain
  • Depression

Pregnancy and Lactation:

  • Pregnancy: Cildip should not be used during pregnancy, especially in women who may become pregnant. It is essential to consult a doctor for an alternative treatment if you are pregnant or planning to become pregnant.

  • Lactation: Cildip should be avoided in breastfeeding mothers. If the medication is essential, breastfeeding should be discontinued. Always consult your doctor before taking Cildip during breastfeeding.
