Description

Indications

Cildip is used for the management of hypertension, aiming to protect vital organs. It has been shown to be beneficial for elderly patients and those suffering from diabetes and albuminuria. Cildip is also gaining importance in the treatment of patients with chronic kidney disease.

Hypertension, commonly known as high blood pressure, occurs when there is an increased force of blood against the walls of the arteries. This elevated pressure can put strain on the blood vessels, leading to potential damage over time.

Pharmacology

Cildip contains Cilnidipine, a dihydropyridine calcium channel blocker. It works by binding to specific sites on the L-type voltage-gated calcium channels, inhibiting the influx of calcium (Ca2+) into the smooth muscle cells of the blood vessels. This results in relaxation of the vascular smooth muscle and promotes vasodilation, helping to lower blood pressure.

Cilnidipine also targets N-type calcium channels in the membranes of sympathetic nerve cells, reducing the release of norepinephrine. This prevents an increase in heart rate that often follows a drop in blood pressure, making it particularly effective in maintaining stable blood pressure levels.

Dosage & Administration

• Adults: Take 5-10 mg once daily after breakfast. The maximum recommended dose is 20 mg daily.

• Pediatric use: The safety and efficacy of Cilnidipine in children have not been established.

• Elderly use: Elderly patients may be more susceptible to low blood pressure, so it is advisable to start with the lowest dose (5 mg).

Drug Interactions

Cildip may interact with other antihypertensive medications, antipsychotics, and drugs that can cause hypotension. Additionally, it may interact with drugs like quinidine, carbamazepine, phenytoin, rifampicin, cimetidine, and erythromycin. Always inform your doctor about any other medications you are taking.

Contraindications

Cildip should not be used in patients who:

• Are allergic to Cilnidipine or any of its ingredients.
• Have cardiogenic shock, recent myocardial infarction (MI), acute unstable angina, or severe aortic stenosis.

Side Effects

While Cildip is generally well tolerated, some patients may experience:

• Dizziness
• Flushing
• Headaches
• Hypotension
• Peripheral edema (swelling)
• Palpitations
• Gastrointestinal disturbances (e.g., nausea, constipation)
• Increased urination frequency
• Lethargy
• Eye pain
• Depression
  Consult your healthcare provider if you experience any of these symptoms.

Pregnancy & Lactation

Cildip is not recommended for use during pregnancy or in women who may become pregnant. It should also be avoided during breastfeeding unless absolutely necessary. If Cildip must be used, it is advised to discontinue breastfeeding.

Precautions & Warnings

Cildip should be used with caution in patients:

• With severe liver dysfunction.
• With a history of serious side effects related to calcium channel blockers.
• Dosage should be gradually reduced if discontinuation is required, under careful medical supervision.
