Indications
Clavusef 500 mg Tablet is used for the treatment of bacterial infections caused by susceptible organisms. It is effective in treating:

  • Pharyngitis/Tonsillitis caused by Streptococcus pyogenes.
  • Acute bacterial otitis media, which may be caused by Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis (including beta-lactamase producing strains), or Streptococcus pyogenes.
  • Acute bacterial exacerbations of chronic bronchitis.
  • Acute bacterial maxillary sinusitis.
  • Uncomplicated skin and soft tissue infections (SSTIs).
  • Uncomplicated urinary tract infections (UTIs).
  • Uncomplicated gonorrhea.
  • Community-acquired pneumonia.
  • Multidrug-resistant (MDR) Typhoid fever.
  • Early Lyme disease.

Pharmacology
Clavusef 500 mg Tablet is a combination of two active ingredients: Cefuroxime and Clavulanic Acid.

  • Cefuroxime is a second-generation cephalosporin antibiotic that works by inhibiting bacterial cell wall synthesis. It is effective against a wide range of both Gram-positive and Gram-negative bacteria, including many beta-lactamase producing strains.

  • Clavulanic Acid is a beta-lactamase inhibitor derived from Streptomyces clavuligerus. It binds irreversibly to beta-lactamase enzymes, preventing them from deactivating Cefuroxime. This allows Cefuroxime to maintain its antibacterial activity, even against beta-lactam-resistant bacteria.

Dosage and Administration

For Adults (13 years and older):

  • Pharyngitis/Tonsillitis: 250 mg twice daily for 5–10 days.
  • Acute bacterial maxillary sinusitis: 250 mg twice daily for 10 days.
  • Acute bacterial exacerbation of chronic bronchitis: 250-500 mg twice daily for 10 days.
  • Secondary bacterial infections of acute bronchitis: 250-500 mg twice daily for 5–10 days.
  • Uncomplicated skin and soft tissue infections: 250-500 mg twice daily for 10 days.
  • Uncomplicated urinary tract infections: 250 mg twice daily for 7–10 days.
  • Uncomplicated Gonorrhea: 1000 mg as a single dose.
  • Community-acquired pneumonia: 250-500 mg twice daily for 5–10 days.
  • MDR Typhoid Fever: 500 mg twice daily for 10–14 days.
  • Early Lyme disease: 500 mg twice daily for 20 days.

For Pediatric Patients (3 months to 12 years):

  • Pharyngitis/Tonsillitis: 20 mg/kg/day divided into two doses for 5–10 days.
  • Acute otitis media: 30 mg/kg/day divided into two doses for 10 days.
  • Acute bacterial maxillary sinusitis: 30 mg/kg/day divided into two doses for 10 days.
  • Impetigo: 30 mg/kg/day divided into two doses for 10 days.

Clavusef tablets can be taken with or without food.

Interactions

  • Probenecid may increase the serum concentration of Cefuroxime by 50%.
  • Medications that reduce gastric acidity may lower the bioavailability of Cefuroxime, potentially reducing its effectiveness.

Contraindications
Clavusef should not be used in patients with:

  • Known hypersensitivity to cephalosporins.
  • A history of Pseudomembranous Colitis.

Side Effects
Common side effects include:

  • Nausea, vomiting, diarrhea, abdominal pain, or discomfort.
  • As with other antibiotics, long-term use may lead to overgrowth of non-susceptible microorganisms.

Rare side effects (less than 0.2%) may include:

  • Renal dysfunction, anaphylaxis, angioedema, pruritus, rash, and serum sickness-like urticaria.

Pregnancy & Lactation

  • Cefuroxime-Clavulanic Acid combination can be used safely in pregnancy, particularly for treating urinary and other infections in later stages.
  • It is excreted into breast milk in small amounts; therefore, nursing mothers should use caution.

Precautions & Warnings

  • Use with caution in patients on diuretics or those with a history of colitis.
  • If you experience any signs of allergic reactions such as a rash or difficulty breathing, stop using the medication and contact your doctor immediately.
