Indications

Climycin is indicated for the treatment of various infections caused by susceptible anaerobic bacteria or specific gram-positive bacteria strains, such as Streptococci, Staphylococci, and Pneumococci. These infections include:

  • Upper respiratory infections
  • Lower respiratory infections
  • Skin and soft tissue infections
  • Bone and joint infections
  • Pelvic infections
  • Intra-abdominal infections
  • Septicemia and endocarditis
  • Dental infections

Climycin can also be used in combination with quinine or amodiaquine as an alternative therapy for multi-drug resistant Plasmodium falciparum infections.

Description

Climycin is a semisynthetic lincosamide antibiotic that is effective in treating infections caused by susceptible microorganisms. It is derived from lincomycin.

Climycin is active against:

  • Aerobic Gram-positive cocci, such as Staphylococcus aureus, Staphylococcus epidermidis (both penicillinase and non-penicillinase producing strains), Streptococci, and Pneumococci.

  • Anaerobic Gram-negative bacilli, including Bacteroides and Fusobacterium species.

  • Anaerobic Gram-positive non-spore forming bacilli, such as Propionibacterium, Eubacterium, and Actinomyces species.

  • Anaerobic and microaerophilic Gram-positive cocci, including Peptococcus, Peptostreptococcus, microaerophilic Streptococci, and Clostridium perfringens.


Dosage & Administration

Clindamycin Capsules

  • For Serious Infections: 150 mg to 300 mg every 6 hours.
  • For More Severe Infections: 300 mg to 450 mg every 6 hours.

It is recommended to take Clindamycin capsules with a full glass of water to prevent possible esophageal irritation.

Research indicates that Clindamycin 300 mg capsules maintain plasma concentrations above the MIC90 for over 12 hours, supporting a twice-daily dosing regimen, particularly for skin, soft tissue, and respiratory infections. However, for bone and joint infections or diabetic foot infections, the recommended dose may increase to 300 mg 3–4 times a day.

Clindamycin Powder for Oral Solution

  • For Serious Infections: 8-12 mg/kg/day, divided into 3 or 4 equal doses.
  • For Severe Infections: 13-16 mg/kg/day, divided into 3 or 4 equal doses.
  • For More Severe Infections: 17-25 mg/kg/day, divided into 3 or 4 equal doses.

For pediatric patients weighing 10 kg or less, 1/2 teaspoon (37.5 mg) three times a day is the minimum recommended dose.

Clindamycin IV/IM Injection

  • For Adults:

    • Serious infections: 600-1200 mg/day in 2-4 equal doses.
    • More severe infections: 1200-2700 mg/day in 2-4 equal doses.
    • In life-threatening situations, doses may need to be increased, with intravenous doses of up to 4800 mg/day.
  • For Neonates (less than 1 month): 15-20 mg/kg/day in 3-4 equal doses.

  • For Pediatric Patients (1 month to 16 years): 20-40 mg/kg/day in 3 or 4 equal doses.

Parenteral therapy may be switched to capsules once the condition stabilizes, as determined by the prescribing physician. In cases of β-hemolytic streptococcal infections, treatment should continue for at least 10 days.

Drug Interactions

Climycin can enhance the effects of neuromuscular blocking agents, so caution is required when used with such agents. Additionally, there is antagonism between Climycin and erythromycin in vitro, meaning these two medications should not be used together.


Contraindications

Climycin should not be used in patients with a known hypersensitivity to clindamycin or any of the components of this medication.


Side Effects

Possible side effects of Climycin include:

  • Abdominal pain
  • Esophagitis and esophageal ulcers
  • Nausea, vomiting, and diarrhea
  • Pruritus (itching), skin rashes, and urticaria (hives)

Pregnancy & Lactation

Climycin is classified as Pregnancy Category B. While it does cross the placenta, the concentration in amniotic fluid is approximately 30% of the maternal concentration. Climycin should only be used during pregnancy if clearly necessary. The drug is also present in breast milk, so it is not recommended for breastfeeding mothers unless absolutely necessary.


Precautions & Warnings

Climycin should be used cautiously in individuals with a history of gastrointestinal issues, especially colitis.


Use in Special Populations

  • Newborns and Infants: Monitoring of organ functions is advised when Climycin is administered to newborns and infants (birth to 16 years).
  • Geriatric Use: No dose adjustment is necessary for elderly patients.

Overdose Effects

Overdose with Climycin is rare. Symptoms are generally similar to those observed with normal doses, but unexpected reactions may occur. Hemodialysis and peritoneal dialysis are not effective in removing Climycin from the bloodstream. In the case of an overdose, simple gastric lavage is recommended, as there is no specific antidote.

Reconstitution Instructions

  • For Oral Solution: Shake the bottle to loosen the powder. Add 80 ml of boiled and cooled water to the bottle in two portions. Shake well after each addition to ensure complete dissolution. The reconstituted solution should be used within 2 weeks if kept at room temperature.

  • For Intravenous Injection: Clindamycin must be diluted prior to intravenous administration. The concentration should not exceed 18 mg/ml. The infusion rate should not exceed 30 mg per minute, and doses over 1200 mg in a single 1-hour infusion are not recommended. Single intramuscular injections greater than 600 mg are also not recommended.


Dilution & Compatibility

Studies show that Climycin is compatible with sodium chloride, glucose, calcium, potassium, and vitamin B complex solutions, as well as antibiotics like cephalothin, kanamycin, gentamicin, penicillin, and carbenicillin. Physical and biological compatibility was observed over 24 hours at room temperature.

Stability of Diluted Solutions:

  • At room temperature (25°C): Stable for 16 days.
  • In refrigeration (4°C): Stable for 32 days.
