Description
Indications
Clindacin is an effective treatment option for infections caused by susceptible anaerobic bacteria or specific gram-positive bacteria strains, including Streptococci, Staphylococci, and Pneumococci. The infections it can treat include:
- Upper respiratory infections
- Lower respiratory infections
- Skin and soft tissue infections
- Bone and joint infections
- Pelvic infections
- Intra-abdominal infections
- Septicemia and endocarditis
- Dental infections
Clindacin is also used in combination with quinine or amodiaquine as an alternative therapy for treating multi-drug resistant Plasmodium falciparum infections.

Description
Clindacin is a lincosamide antibiotic that is effective in treating infections caused by microorganisms sensitive to the drug. It is a semisynthetic antibiotic derived from lincomycin.
Clindacin works against:

Aerobic Gram-positive cocci, including Staphylococcus aureus, Staphylococcus epidermidis (both penicillinase and non-penicillinase producing strains), Streptococci, and Pneumococci.

Anaerobic Gram-negative bacilli, such as Bacteroides and Fusobacterium species.

Anaerobic Gram-positive non-spore-forming bacilli, including Propionibacterium, Eubacterium, and Actinomyces species.

Anaerobic and microaerophilic Gram-positive cocci, such as Peptococcus, Peptostreptococcus, microaerophilic Streptococci, and Clostridium perfringens.
Dosage & Administration
Clindacin Capsules
- For Serious Infections: 150 mg to 300 mg every 6 hours.
- For More Severe Infections: 300 mg to 450 mg every 6 hours.
To minimize the risk of esophageal irritation, take Clindacin capsules with a full glass of water.
Studies have shown that Clindacin 300 mg capsules maintain plasma concentrations above the MIC90 for over 12 hours, supporting a twice-daily dosing regimen, especially for skin, soft tissue, and respiratory infections. For bone and joint infections or diabetic foot infections, the dose may be increased to 300 mg 3-4 times daily.
Clindacin Powder for Oral Solution
- For Serious Infections: 8-12 mg/kg/day, divided into 3 or 4 equal doses.
- For Severe Infections: 13-16 mg/kg/day, divided into 3 or 4 equal doses.
- For More Severe Infections: 17-25 mg/kg/day, divided into 3 or 4 equal doses.
For pediatric patients weighing 10 kg or less, 1/2 teaspoon (37.5 mg) three times a day is the minimum recommended dose.
Clindacin IV/IM Injection
- For Adults:
- Serious infections from aerobic gram-positive cocci or susceptible anaerobes: 600-1200 mg/day in 2-4 equal doses.
- For more severe infections: 1200-2700 mg/day in 2-4 equal doses.
In critical situations, the dose may need to be adjusted, and intravenous doses up to 4800 mg/day have been used. Single intramuscular injections of more than 600 mg are not recommended.
- For Neonates (under 1 month): 15-20 mg/kg/day in 3-4 equal doses.
- For Pediatric Patients (1 month to 16 years): 20-40 mg/kg/day in 3-4 equal doses, with higher doses for more severe infections.
Parenteral therapy may be switched to Clindacin capsules as the patient’s condition improves, based on the physician’s judgment. For β-hemolytic streptococcal infections, treatment should continue for a minimum of 10 days.

Drug Interactions
Clindacin may enhance the effects of neuromuscular blocking agents, so caution is necessary when used alongside such drugs. In vitro studies show that Clindacin and erythromycin may antagonize each other’s effects, and therefore, these drugs should not be administered together.
Contraindications
Clindacin should not be used by individuals who are hypersensitive to clindamycin or any of its ingredients.
Side Effects
Possible side effects of Clindacin include:
- Abdominal pain
- Esophagitis and esophageal ulcers
- Nausea, vomiting, and diarrhea
- Itching, skin rashes, and urticaria (hives)
Pregnancy & Lactation
Clindacin is classified as Pregnancy Category B. It crosses the placenta, with amniotic fluid concentrations approximately 30% of the maternal levels after multiple doses. Use Clindacin during pregnancy only when clearly needed. It is also known to pass into breast milk, so it should not be used by nursing mothers unless necessary.
Precautions & Warnings
Clindacin should be prescribed with caution in individuals who have a history of gastrointestinal diseases, particularly colitis.
Use in Special Populations
- Newborns and Infants: Appropriate monitoring of organ functions is essential when administering Clindacin to newborns and infants (birth to 16 years).
- Geriatric Use: No dose adjustment is necessary for elderly patients.
Overdose Effects
Overdose with Clindacin is rare but could lead to adverse reactions similar to those experienced at normal doses. Unexpected side effects may occur. Hemodialysis and peritoneal dialysis are not effective in removing Clindacin from the bloodstream. Overdose should be treated with gastric lavage, and there is no known specific antidote.

Reconstitution Instructions

For Oral Solution: Shake the bottle to loosen the powder. Add 80 ml of boiled and cooled water to the bottle in two portions. Shake well after each addition until the powder dissolves completely. Store the reconstituted solution tightly sealed, and use it within 2 weeks if kept at room temperature.

For IV Injection: Clindacin phosphate should be diluted before intravenous use. The concentration in the diluent should not exceed 18 mg/ml, and the infusion rate should not exceed 30 mg/min. Avoid administering more than 1200 mg in a single 1-hour infusion. Single intramuscular injections over 600 mg are not recommended.
Dilution & Compatibility
Studies indicate that Clindacin is compatible with sodium chloride, glucose, calcium, potassium, and vitamin B complex solutions, as well as antibiotics such as cephalothin, kanamycin, gentamicin, penicillin, and carbenicillin. The drug remains stable when mixed with these solutions at room temperature for up to 24 hours.
Stability of Diluted Clindacin Solutions:
- At Room Temperature (25°C): Stable for 16 days.
- Refrigerated (4°C): Stable for 32 days.
