Description
Indications
Clindacin is indicated for the treatment of various infections caused by susceptible anaerobic bacteria or specific strains of gram-positive bacteria such as Streptococci, Staphylococci, and Pneumococci. The conditions it treats include:
- Upper respiratory tract infections
- Lower respiratory tract infections
- Skin and soft tissue infections
- Bone and joint infections
- Pelvic infections
- Intra-abdominal infections
- Septicemia and endocarditis
- Dental infections
Additionally, Clindacin is used as an alternative therapy in combination with quinine or amodiaquine for treating multi-drug resistant Plasmodium falciparum infections.

Description Clindacin is a lincosamide antibiotic that effectively treats infections caused by susceptible microorganisms. It is a semisynthetic antibiotic derived from lincomycin, offering potent antibacterial properties.
Spectrum of Activity:
- Aerobic gram-positive cocci: Staphylococcus aureus, Staphylococcus epidermidis (both penicillinase-producing and non-penicillinase-producing strains), Streptococci, Pneumococci.
- Anaerobic gram-negative bacilli: Bacteroides species, Fusobacterium species.
- Anaerobic gram-positive non-spore-forming bacilli: Propionibacterium species, Eubacterium species, Actinomyces species.
- Anaerobic and microaerophilic gram-positive cocci: Peptococcus species, Peptostreptococcus species, Microaerophilic streptococci, Clostridium perfringens.
Dosage & Administration
Clindamycin Capsules:
- For serious infections: 150 mg to 300 mg every six hours.
- For more severe infections: 300 mg to 450 mg every six hours.
- Clindacin capsules should be taken with a full glass of water to prevent esophageal irritation.
- Studies suggest that Clindamycin 300 mg provides effective plasma concentration over MIC90 for more than 12 hours, supporting a twice-daily dosage in SSTIs and RTIs. However, for bone and joint infections or diabetic foot infections, the recommended dose is 300 mg taken three to four times daily.
Clindamycin Powder for Oral Solution:
- For serious infections: 8-12 mg/kg/day divided into 3-4 doses.
- For severe infections: 13-16 mg/kg/day divided into 3-4 doses.
- For more severe infections: 17-25 mg/kg/day divided into 3-4 doses.
- In pediatric patients weighing 10 kg or less, the minimum recommended dose is 37.5 mg (1/2 teaspoon) three times daily.
Clindamycin IV/IM Injection:
- Adults:
- Serious infections: 600-1200 mg/day divided into 2-4 doses.
- More severe infections: 1200-2700 mg/day divided into 2-4 doses.
- In life-threatening situations, doses may be increased. IV doses up to 4800 mg/day have been used.
- Single IM injections should not exceed 600 mg.
- Neonates (less than 1 month): 15-20 mg/kg/day in 3-4 divided doses.
- Pediatric Patients (1 month to 16 years): 20-40 mg/kg/day divided into 3-4 doses, with higher doses used for severe infections.
- IV therapy may be switched to oral Clindacin when appropriate.
- Treatment for beta-hemolytic streptococcal infections should continue for at least 10 days.

Interaction
- Clindacin enhances the action of neuromuscular blocking agents, requiring caution when used alongside such medications.
- Antagonism between Clindacin and erythromycin has been observed in vitro, so concurrent administration should be avoided.
Contraindications
- Clindacin is contraindicated in individuals with known hypersensitivity to clindamycin or any of its components.
Side Effects Reported adverse effects include:
- Abdominal pain
- Nausea, vomiting, diarrhea
- Esophagitis and esophageal ulceration
- Skin reactions such as rash, pruritus, urticaria
Pregnancy & Lactation
- Pregnancy (Category B): Clindamycin crosses the placenta. After multiple doses, amniotic fluid concentrations reach approximately 30% of maternal levels. It should only be used during pregnancy if clearly necessary.
- Lactation: Clindamycin has been detected in breast milk, so it should be used with caution in nursing mothers.
Precautions & Warnings
- Clindacin should be prescribed with caution in individuals with a history of gastrointestinal disease, particularly colitis.
- Avoid prolonged use to prevent bacterial resistance.
Use in Special Populations
- Neonates and Infants: Requires careful monitoring of organ function.
- Geriatric Use: No specific dose adjustment is required for elderly patients.
Overdose Effects
- Overdose cases are rare but may present symptoms similar to normal dose side effects.
- Hemodialysis and peritoneal dialysis do not effectively remove Clindacin from the serum.
- Overdose treatment includes gastric lavage and symptomatic management.
Therapeutic Class Macrolides
Reconstitution
- Oral Solution: Shake the bottle to loosen the powder, add 80 mL of cooled boiled water in two portions, shaking well after each addition. Keep the bottle tightly closed. The reconstituted solution should be used within 2 weeks at room temperature.
- IV Injection: Clindacin phosphate must be diluted before IV use, with a maximum concentration of 18 mg/mL and an infusion rate not exceeding 30 mg/min. Infusions over 1200 mg should be administered over at least 1 hour.
- IM Injection: Single IM doses should not exceed 600 mg. No dilution is required for IM administration.
- Compatibility: Clindacin phosphate remains stable for 24 hours at room temperature when mixed with IV solutions containing sodium chloride, glucose, calcium, potassium, and vitamin B complex. It is compatible with antibiotics such as cephalothin, kanamycin, gentamicin, penicillin, and carbenicillin.
