Description

Indications Comet XR tablets are prescribed for managing type 2 diabetes mellitus, particularly in overweight patients when diet and exercise alone fail to provide adequate glycemic control.

  • Adults: Comet XR can be used as monotherapy or combined with other oral antidiabetic medications or insulin.
  • Children (10 years and older) and adolescents: Comet XR can be used alone or in combination with insulin.

Studies have shown that metformin, as a first-line treatment after dietary failure, reduces complications associated with type 2 diabetes in overweight adults.

Pharmacology Metformin, a biguanide-class oral antihyperglycemic agent, helps manage type 2 diabetes by lowering basal and postprandial plasma glucose levels. Unlike sulfonylureas, it does not induce hypoglycemia. Its mechanism of action includes:

  • Reducing hepatic glucose production
  • Decreasing intestinal glucose absorption
  • Improving insulin sensitivity by enhancing peripheral glucose uptake and utilization

Dosage & Administration Metformin Immediate-Release Tablet:

  • Adults: The usual starting dose is 500 mg twice daily or 850 mg once daily with meals. The dose may be increased weekly by 500 mg or biweekly by 850 mg, up to a maximum of 2000 mg per day, divided into multiple doses. For patients needing further glycemic control, the daily dose may go up to 2550 mg.
  • Children: The initial dose is 500 mg twice daily with meals, with gradual increments of 500 mg weekly, up to a maximum of 2000 mg per day in divided doses.

Metformin Extended-Release Tablet:

  • Adults: The typical starting dose is 500 mg once daily with the evening meal. The dose can be increased weekly by 500 mg, up to a maximum of 2000 mg once daily or 1000 mg twice daily with meals. Patients switching from immediate-release metformin should not exceed the recommended daily dose.
  • Children: Extended-release tablets have not been studied in pediatric patients.

Patients with Renal Impairment:

  • Metformin is contraindicated in patients with an eGFR below 30 mL/min/1.73 m².
  • The benefits and risks should be assessed in patients with an eGFR between 30-45 mL/min/1.73 m².

Drug Interactions

  • Nifedipine may enhance the absorption of Comet XR.
  • Comet XR has minimal effects on nifedipine.
  • Cationic drugs (e.g., amiloride, digoxin, morphine, quinidine, trimethoprim, vancomycin) may compete for renal tubular transport, potentially altering metformin clearance.
  • Hyperglycemia-inducing drugs, such as corticosteroids, diuretics, estrogens, and thyroid medications, may reduce the effectiveness of Comet XR.

Contraindications Comet XR is not recommended for patients with:

  • Hypersensitivity to metformin or its excipients
  • Acute metabolic acidosis (e.g., lactic acidosis, diabetic ketoacidosis)
  • Severe renal impairment (GFR <30 mL/min)
  • Conditions affecting renal function (e.g., dehydration, severe infection, shock)
  • Diseases causing tissue hypoxia (e.g., heart or respiratory failure, myocardial infarction)
  • Liver dysfunction, acute alcohol intoxication, or alcoholism

Side Effects

  • Blood Disorders: Hemolytic anemia (rare)
  • Metabolism: Rare cases of lactic acidosis and decreased vitamin B12 absorption, potentially leading to megaloblastic anemia or peripheral neuropathy
  • Nervous System: Common reports of taste disturbances; encephalopathy (unknown frequency)
  • Gastrointestinal: Nausea, vomiting, diarrhea, abdominal pain, and appetite loss (common, particularly at treatment initiation)
  • Liver: Rare cases of hepatic dysfunction, reversible upon discontinuation
  • Skin: Rare allergic reactions such as erythema, pruritus, or urticaria

Use During Pregnancy & Lactation

  • Pregnancy: Uncontrolled diabetes during pregnancy increases the risk of congenital anomalies and perinatal mortality. Insulin is the preferred treatment to maintain normal blood glucose levels.
  • Breastfeeding: Metformin is excreted in breast milk. While no adverse effects have been observed in infants, due to limited data, breastfeeding is not recommended during metformin therapy.

Precautions & Warnings

  • Comet XR is primarily excreted by the kidneys. The risk of lactic acidosis increases with declining renal function; thus, renal function should be regularly monitored.
  • Vitamin B12 levels should be assessed annually in long-term users due to potential absorption reduction.
  • Therapy should be discontinued 2-3 days before surgery or contrast imaging procedures and resumed only after renal function is reassessed.
  • Combination therapy with sulfonylureas or insulin may increase the risk of hypoglycemia.
  • Cimetidine may reduce metformin clearance, warranting dose adjustments.

Use in Special Populations

  • Elderly: Due to reduced renal function, dosage should be adjusted accordingly, with frequent renal function monitoring.
  • Pediatric Patients: Metformin use in children aged 10-12 years requires caution. Long-term effects on growth and puberty are unknown.
  • Renal Function Monitoring:
    • At least annually for patients with normal renal function
    • Two to four times per year for those with borderline renal function or elderly patients

Overdose Effects

  • Severe overdose may lead to lactic acidosis, requiring urgent medical intervention. Hemodialysis is the most effective method for metformin and lactate removal.
