Description

Indications Comprid XR is an oral antidiabetic medication belonging to the sulfonylurea class, designed to help lower blood sugar levels. It is prescribed for adults with type 2 diabetes mellitus when diet, exercise, and weight management alone are insufficient in maintaining optimal blood sugar control.

Pharmacology Comprid XR contains Gliclazide, a second-generation sulfonylurea that exhibits hypoglycemic properties. It works by stimulating the release of insulin from pancreatic beta cells, facilitating calcium ion (Ca+2) transport across cell membranes, and reducing hepatic glucose production. Additionally, it may have beneficial hematological effects.

Dosage & Administration

Film-Coated Tablet: The typical starting dose is 40-80 mg per day, with a maximum dose of 320 mg daily, divided into multiple doses. It should be taken before meals. Gliclazide is not recommended for children as it is contraindicated in juvenile-onset diabetes.

Modified-Release (MR) Tablet:

  • Always take this medication exactly as prescribed by your doctor or pharmacist.
  • The dose is determined based on blood and, if necessary, urine sugar levels.
  • Adjustments in dosage may be required due to changes in weight, lifestyle, or stress levels.
  • The recommended dose is one to four tablets (maximum 120 mg) in a single daily dose at breakfast.
  • Swallow the tablet whole with a glass of water. Do not chew or crush it. Always consume a meal after taking this medication.

If combined with metformin, alpha-glucosidase inhibitors, thiazolidinediones, DPP-4 inhibitors, GLP-1 receptor agonists, or insulin, the dosage of each medication will be individually determined by your doctor.

Overdose Management Taking more than the prescribed dose may result in hypoglycemia (low blood sugar). Symptoms include dizziness, confusion, sweating, and rapid heartbeat. Immediate treatment involves consuming sugar (such as sugar cubes or sweet drinks), followed by a substantial meal. If unconscious, seek medical attention immediately.

Missed Dose If you miss a dose, take it as soon as possible. If it is nearly time for the next dose, skip the missed dose. Do not double up to compensate for a missed dose.

Discontinuation Diabetes treatment is typically lifelong. Do not stop taking Comprid XR without consulting your doctor, as this could lead to high blood sugar (hyperglycemia) and increase the risk of diabetes-related complications.

Drug Interactions Certain medications can enhance or weaken the effects of Comprid XR:

Medications that may enhance the blood sugar-lowering effect:

  • Other diabetes medications (e.g., GLP-1 receptor agonists, insulin)
  • Certain antibiotics (e.g., sulfonamides, clarithromycin)
  • Blood pressure medications (e.g., beta-blockers, ACE inhibitors)
  • Antifungal medications (e.g., miconazole, fluconazole)
  • Anti-ulcer drugs (e.g., H2 receptor antagonists)
  • Monoamine oxidase inhibitors (MAOIs) for depression
  • Anti-inflammatory drugs (e.g., ibuprofen, phenylbutazone)
  • Alcohol

Medications that may weaken the blood sugar-lowering effect:

  • Certain psychiatric medications (e.g., chlorpromazine)
  • Corticosteroids
  • Asthma medications (e.g., intravenous salbutamol, ritodrine, terbutaline)
  • Medications for gynecological conditions (e.g., danazol)
  • St. John’s Wort

Comprid XR may also increase the effects of anticoagulants such as warfarin.

Avoid alcohol consumption as it may unpredictably affect blood sugar levels.

Precautions & Warnings

  • Follow the prescribed treatment plan, including medication, diet, and exercise.
  • Regular monitoring of blood sugar levels and glycated hemoglobin (HbA1c) is necessary.
  • Increased risk of hypoglycemia, particularly in the first few weeks of treatment.
  • Symptoms of hypoglycemia include dizziness, sweating, weakness, and confusion.
  • Symptoms of hyperglycemia include excessive thirst, frequent urination, dry skin, and fatigue.
  • Special caution is required if you have liver or kidney impairment.
  • If undergoing surgery or experiencing severe stress, insulin therapy may be temporarily required.
  • Individuals with G6PD deficiency should consult their doctor before taking this medication.

Contraindications Do not take Comprid XR if:

  • You are allergic to Gliclazide, other sulfonylureas, or sulfonamide derivatives.
  • You have type 1 diabetes.
  • You have diabetic ketoacidosis, diabetic coma, or pre-coma.
  • You have severe kidney or liver disease.
  • You are taking antifungal medications.
  • You are breastfeeding.

Side Effects Common side effects include:

  • Hypoglycemia (low blood sugar), which may cause dizziness, sweating, weakness, and confusion.
  • Gastrointestinal issues such as nausea, vomiting, indigestion, diarrhea, or constipation.
  • Liver dysfunction (rare), which may present as yellowing of the skin and eyes.
  • Skin reactions such as rash, redness, itching, hives, or, in rare cases, severe allergic reactions.
  • Blood disorders (rare) that may cause unusual bleeding, paleness, or fever.
  • Vision changes at the start of treatment due to fluctuating blood sugar levels.

Pregnancy & Lactation Comprid XR is not recommended for use during pregnancy or while breastfeeding. If you are pregnant or planning to conceive, consult your doctor for alternative treatment options.

Driving & Operating Machinery Low or high blood sugar levels may affect concentration and reaction times, potentially making activities such as driving or operating machinery unsafe. Discuss your condition with your doctor if you experience frequent hypoglycemic episodes.
