Description

Indications
Dermasim VT rectal preparation is used for the treatment of vaginal itching, burning, and discharge commonly associated with recurrent vaginal yeast infections (Vaginal Candidiasis). It is also effective in treating vaginitis caused by Candida species and other mycotic infections that may be complicated by other microorganisms sensitive to the medication.

Pharmacology
Clotrimazole is a broad-spectrum antifungal agent that targets a variety of fungal species, including dermatophytes, yeasts, molds, and bacteroids. It also demonstrates activity against certain gram-positive bacteria, particularly Staphylococci, and Trichomonas. Clotrimazole, similar to other imidazoles, works by disrupting the integrity of fungal cell walls by interacting with the cell wall lipids. At lower concentrations, it inhibits the synthesis of ergosterol, a key component of fungal cell membranes. At higher doses, Clotrimazole causes additional membrane damage independent of sterol synthesis.

Dosage

• Clotrimazole 100 mg & 200 mg vaginal preparations:
  • For First Infections: A 3-day course is generally sufficient to treat Candida vaginitis. On three consecutive nights, insert 1 Clotrimazole 200 mg vaginal tablet as deeply as possible into the vagina. This is most effectively done while lying on the back with the knees slightly bent.
  • For Re-infections: A 6-day course is typically adequate. For mixed infections or recurrent Candida vaginitis, insert 1 Clotrimazole 100 mg vaginal tablet deeply into the vagina every night for 6 consecutive nights. If necessary, use 2 Clotrimazole 100 mg tablets per day (1 in the morning and 1 in the evening) or 1 Clotrimazole 200 mg tablet daily for 6-12 days. Ensure the treatment does not coincide with the menstrual period and is completed before menstruation begins. For preventing reinfection, the partner should also be treated locally with Clotrimazole cream.
  • Clotrimazole 500 mg vaginal preparation: This is a single-dose treatment. Insert one 500 mg vaginal tablet as deeply as possible into the vagina, best achieved while lying on your back with knees bent. The treatment should be timed to avoid the menstrual period. For prevention of reinfection, the partner should be treated locally with Clotrimazole cream.

Clotrimazole vaginal preparations are colorless and will not stain underwear.

Administration

1. Pull the plunger of the applicator until it stops.
2. Place a vaginal tablet into the applicator.
3. Insert the applicator carefully into the vagina, as deeply as possible (best done while lying on your back).
4. Push the plunger until it stops, ensuring the tablet is deposited inside the vagina.
5. After use, remove the applicator, and pull out the plunger completely.
6. Clean the applicator with warm soapy water, rinse it, and dry thoroughly.

Contraindications
This product should not be used by individuals who are hypersensitive to any of its components.

Side Effects
Since Dermasim VT is minimally absorbed through the vaginal lining, it does not typically produce systemic effects. Most users tolerate the product well. However, in rare cases, local irritation or a mild burning sensation may occur, but these symptoms are generally not harmful.

Pregnancy & Lactation
Clotrimazole should only be used during pregnancy when deemed necessary by a physician. Clotrimazole has minimal systemic absorption when applied topically, and studies in women during the second or third trimesters have shown no adverse effects. It is not known if Clotrimazole passes into breast milk. The use of Clotrimazole suppositories during pregnancy is possible but should be done under a doctor’s supervision.
