Description
Dimerol MR 60 mg Tablet
Indications
Dimerol MR is an oral antidiabetic medication from the sulfonylurea group, designed to lower blood sugar levels. It is prescribed for adults with type 2 diabetes mellitus when diet, exercise, and weight control alone are insufficient to maintain proper blood sugar levels.
Pharmacology
Gliclazide, the active ingredient in Dimerol MR, is a second-generation sulfonylurea with hypoglycemic and potential hematological benefits. It stimulates insulin release from pancreatic β-cells by enhancing calcium transport across β-cell membranes, reducing hepatic glucose production.
Dosage & Administration
- Film-coated tablet: The usual starting dose ranges from 40 mg to 80 mg daily. The dose can be increased up to 320 mg daily in divided doses if required. It should be taken before meals. This medication is not suitable for children with juvenile-onset diabetes.
- Modified-release tablet: The dosage should be determined by a doctor based on blood and urine sugar levels. External factors such as weight loss, lifestyle changes, and stress may require dose adjustments. The recommended daily dose is between one to four tablets (maximum 120 mg) taken once daily at breakfast. Tablets must be swallowed whole with water and not chewed or crushed.
- In combination therapy with metformin, alpha-glucosidase inhibitors, thiazolidinediones, DPP-4 inhibitors, GLP-1 receptor agonists, or insulin, the doctor will determine the appropriate dosage.
- In case of overdose, immediate medical attention is required. Symptoms of overdose include hypoglycemia, which can be managed by consuming sugar or sugary drinks followed by a substantial meal. Severe cases require urgent medical intervention.
- If a dose is missed, take the next scheduled dose. Do not double up on doses.
- Discontinuing treatment without medical advice may lead to high blood sugar (hyperglycemia) and increase diabetes-related complications.
Drug Interactions
- The blood sugar-lowering effect of Dimerol MR may be enhanced by other antidiabetic drugs, antibiotics (sulfonamides, clarithromycin), antihypertensives (beta-blockers, ACE inhibitors), antifungals (miconazole, fluconazole), H2 receptor antagonists, MAO inhibitors, pain relievers (phenylbutazone, ibuprofen), and alcohol.
- The hypoglycemic effect may be reduced by central nervous system medications (chlorpromazine), corticosteroids, asthma medications (intravenous salbutamol, ritodrine, terbutaline), danazol, and St. John’s Wort.
- Fluoroquinolone antibiotics may cause blood sugar fluctuations, especially in elderly patients.
- Dimerol MR may increase the effect of anticoagulants like warfarin.
- Alcohol consumption should be avoided, as it can unpredictably affect blood sugar levels.
Contraindications
Dimerol MR should not be taken if:
- You are allergic to gliclazide, other sulfonylureas, or related hypoglycemic sulfonamides.
- You have type 1 diabetes.
- You have diabetic ketoacidosis, diabetic coma, or pre-coma.
- You have severe kidney or liver disease.
- You are taking antifungal medications.
- You are breastfeeding.
Side Effects
- Common: Hypoglycemia (low blood sugar) may occur, leading to symptoms like dizziness, hunger, sweating, confusion, or even unconsciousness in severe cases.
- Liver Disorders: Rare cases of abnormal liver function may cause jaundice. Seek immediate medical attention if this occurs.
- Skin Reactions: Rash, itching, redness, hives, and swelling may develop. Severe allergic reactions require urgent medical care.
- Blood Disorders: Reduced blood cell count may lead to paleness, prolonged bleeding, bruising, sore throat, and fever, which typically resolve upon discontinuation.
- Digestive Issues: Nausea, vomiting, indigestion, diarrhea, and constipation can occur, but taking the medication with food reduces these effects.
- Eye Disorders: Vision changes may occur at the beginning of treatment due to fluctuations in blood sugar levels.
Pregnancy & Lactation
Dimerol MR is not recommended during pregnancy. Pregnant or breastfeeding women should consult a doctor before taking this medication.
Precautions & Warnings
- Follow your prescribed treatment plan, including dietary recommendations, physical activity, and weight management.
- Regular monitoring of blood sugar levels (blood and urine tests) and HbA1c is essential.
- The risk of hypoglycemia is higher in the early weeks of treatment, requiring close medical supervision.
- Factors that may increase the risk of hypoglycemia include skipping meals, fasting, malnutrition, alcohol consumption, intense physical activity without proper carbohydrate intake, taking high doses, and certain hormone disorders.
- In case of hypoglycemia, consume sugar (glucose tablets, sugar cubes, sweet juice, or tea) and seek medical help if symptoms persist.
- Elderly patients and those on central nervous system medications or beta-blockers may not notice hypoglycemia symptoms as quickly.
- Stressful situations (illness, surgery, fever) may require temporary insulin therapy.
- Symptoms of hyperglycemia include thirst, frequent urination, dry skin, and fatigue. Seek medical guidance if symptoms appear.
- Patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency should consult a doctor before use, as it may cause hemolytic anemia.
