Indications
Domilux is primarily indicated for the treatment of:

  • Dyspeptic symptom complex, often linked with delayed gastric emptying, gastroesophageal reflux, and esophagitis, which include:
    • Epigastric fullness, abdominal distension, upper abdominal pain
    • Eructation, flatulence, early satiety
    • Nausea and vomiting
    • Heartburn, with or without regurgitation of gastric contents in the mouth
  • Non-ulcer dyspepsia
  • Acute nausea and vomiting, whether functional, organic, infectious, dietetic, induced by radiotherapy or drug therapy, or linked to migraines
  • Parkinson’s disease, specifically for dopamine-agonist induced nausea and vomiting
  • Radiological studies, to speed up barium transit in follow-through radiological studies.

Pharmacology
Domperidone is a dopamine antagonist that primarily works by blocking dopamine receptors in the Chemoreceptor Trigger Zone (CTZ) and the stomach. Its gastroprokinetic effects are attributed to its inhibition of dopamine receptors, which influence gastrointestinal motility. Due to its limited ability to cross the blood-brain barrier, domperidone causes minimal impact on the central dopaminergic system, thus avoiding psychotropic and neurological side effects. Domperidone restores normal motility and tone in the upper gastrointestinal tract, facilitates gastric emptying, enhances peristalsis in the antrum and duodenum, and regulates pyloric contraction. Additionally, it increases esophageal peristalsis and lowers esophageal sphincter pressure, thus preventing gastric regurgitation.

Dosage & Administration
Domperidone should be taken 15–30 minutes before meals and, if required, before bedtime.

Recommended Dosage:

  • Adults: 10–20 mg (1–2 tablets or 10–20 ml suspension), every 6–8 hours. The maximum daily dose is 80 mg.
  • Children: 2–4 ml suspension/10 kg body weight or 0.4–0.8 ml pediatric drops/10 kg body weight, every 6–8 hours.

For Dyspeptic Symptoms:

  • Adults: 10–20 mg (1–2 tablets or 10–20 ml suspension), every 6–8 hours.
  • Children: 0.2–0.4 mg/kg body weight (2–4 ml suspension/10 kg or 0.4–0.8 ml pediatric drops/10 kg), every 6–8 hours.

For Acute and Sub-Acute Conditions (e.g., Acute Nausea and Vomiting):

  • Adults: 20 mg (2 tablets or 20 ml suspension), every 6–8 hours.
  • Children: 0.2–0.4 mg/kg body weight (2–4 ml suspension/10 kg or 0.4–0.8 ml pediatric drops/10 kg), every 6–8 hours. (Maximum treatment duration for acute nausea and vomiting is 12 weeks).

Rectal Administration (Suppositories):

  • Adults: 30–60 mg every 4–8 hours.
  • Children: The maximum daily dose for children weighing 10–25 kg is 30 mg, which may be divided into multiple doses. The maximum treatment duration is 12 weeks.

Interactions

  • Anticholinergic drugs may reduce the effectiveness of domperidone in treating dyspepsia.
  • Antacids and antisecretory drugs should not be taken simultaneously with domperidone as they reduce its bioavailability.
  • Domperidone is metabolized through the CYP3A4 enzyme, and drugs that inhibit this enzyme (such as azole antifungals, macrolide antibiotics, HIV protease inhibitors, and nefazodone) may increase domperidone levels in the blood.
  • Domperidone’s gastrokinetic properties could potentially affect the absorption of other orally administered medications, particularly sustained-release or enteric-coated formulations.

Contraindications
Domperidone should not be used in patients with:

  • Known hypersensitivity to domperidone
  • Neonates
  • Conditions where gastrointestinal stimulation could be harmful (e.g., gastrointestinal hemorrhage, obstruction, or perforation)
  • Prolactin-releasing pituitary tumors (prolactinoma).

Side Effects
Side effects are rare but may include transient intestinal cramps. Extrapyramidal symptoms are uncommon in adults but may occur in young children, usually reversing once the medication is stopped. Domperidone may elevate plasma prolactin levels, potentially causing conditions like galactorrhea and gynecomastia in rare cases. In infants, neurological side effects may occur due to an immature blood-brain barrier. Allergic reactions like rash and urticaria have also been reported.

Pregnancy & Lactation
Domperidone has not shown teratogenic effects in animal studies. However, it should only be used during the first trimester of pregnancy when the benefits outweigh the risks. Domperidone is excreted in the milk of lactating rats, and while human data shows lower concentrations in breast milk, it is not recommended for breastfeeding mothers unless absolutely necessary. The potential risks should be weighed carefully.

Precautions & Warnings

  • Caution should be exercised in children, as they may be more prone to extrapyramidal reactions.
  • Caution is also recommended in patients with liver impairment, as the drug is metabolized in the liver.

Use in Special Populations

  • Infants: Due to incomplete development of metabolic and blood-brain barrier functions, Domperidone should be used cautiously and under close medical supervision.
  • Liver Disorders: Domperidone is metabolized in the liver and should be used with caution in patients with hepatic impairment.
  • Kidney Disorders: In patients with severe renal insufficiency, the drug’s half-life is prolonged. The dose may need to be adjusted based on severity.

Overdose Effects
Symptoms of overdose may include drowsiness, disorientation, and extrapyramidal reactions, especially in children. Treatment includes activated charcoal and observation. Anticholinergic or antiparkinson drugs may be used to manage extrapyramidal symptoms.
