Indications
Don-A is prescribed for the following conditions:

  • Dyspeptic symptom complex, typically linked to delayed gastric emptying, gastroesophageal reflux, and esophagitis:
    • Epigastric fullness, abdominal distension, and upper abdominal pain
    • Eructation, flatulence, early satiety
    • Nausea and vomiting
    • Heartburn, with or without the regurgitation of gastric contents
  • Non-ulcer dyspepsia
  • Acute nausea and vomiting resulting from functional, organic, infectious, or diet-related causes, or as a side effect of radiotherapy, drug therapy, or migraine
  • Parkinson’s disease-related nausea and vomiting induced by dopamine-agonists
  • Radiological studies to speed up barium transit in follow-through radiological procedures

Pharmacology
Domperidone acts as a dopamine antagonist, mainly blocking dopamine receptors in the Chemoreceptor Trigger Zone (CTZ) and the stomach. This helps in enhancing the motility of the gastrointestinal tract. Due to its minimal penetration of the blood-brain barrier, Domperidone does not significantly affect the dopaminergic receptors in the brain, preventing psychotropic or neurological side effects. Domperidone promotes normal motility and tone in the upper gastrointestinal tract, accelerates gastric emptying, and improves peristalsis in the antrum and duodenum, regulating pyloric contractions. Additionally, it increases esophageal peristalsis and the pressure of the lower esophageal sphincter, which helps prevent gastric content regurgitation.

Dosage & Administration

  • Take Domperidone 15–30 minutes before meals and, if necessary, before going to bed.

Recommended Doses:

  • Adults:
    • 10-20 mg (1-2 tablets or 10-20 ml suspension) every 6–8 hours.
    • Maximum dose: 80 mg per day.
  • Children:
    • 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml pediatric drops/10 kg body weight every 6-8 hours.
  • For Dyspeptic Symptoms:
    • Adults: 10-20 mg (1-2 tablets or 10-20 ml suspension) every 6–8 hours.
    • Children: 0.2-0.4 mg/kg body weight (2-4 ml suspension/10 kg or 0.4-0.8 ml pediatric drops/10 kg) every 6–8 hours.
  • For Acute and Sub-Acute Conditions (primarily acute nausea and vomiting):
    • Adults: 20 mg (2 tablets or 20 ml suspension) every 6-8 hours.
    • Children: 0.2-0.4 mg/kg body weight (2-4 ml suspension/10 kg or 0.4-0.8 ml pediatric drops/10 kg) every 6-8 hours.
      (Treatment duration for acute nausea and vomiting should not exceed 12 weeks.)
  • For Rectal Administration (Suppositories):
    • Adults (including elderly): 30-60 mg every 4-8 hours.
    • Children: Maximum daily dose of 30 mg for children weighing 10–25 kg. The dose can be divided throughout the day as needed.
    • Treatment duration should not exceed 12 weeks.

Interaction

  • Anticholinergic drugs may interfere with the antidyspeptic effect of Domperidone.
  • Antacids and antisecretory drugs should not be used simultaneously as they lower Domperidone’s bioavailability.
  • Drugs that inhibit CYP3A4 (such as azole antifungals, macrolide antibiotics, HIV protease inhibitors, nefazodone, etc.) can increase Domperidone’s plasma levels.
  • Domperidone may alter the absorption of other orally administered drugs, especially those with sustained release or enteric-coated formulations. However, for patients stabilized on digoxin or paracetamol, Domperidone does not affect the blood levels of these drugs.
  • It does not enhance the action of neuroleptics but suppresses unwanted peripheral effects of dopaminergic agonists like bromocriptine and L-dopa without affecting their central actions.

Contraindications

  • Hypersensitivity to Domperidone or any of its components
  • In neonates
  • In conditions where gastrointestinal stimulation is dangerous (e.g., gastrointestinal hemorrhage, obstruction, or perforation)
  • In patients with prolactin-releasing pituitary tumors (prolactinoma)

Side Effects
Side effects are rare but may include transient intestinal cramps. Extrapyramidal symptoms are extremely rare in adults but can occur in young children and resolve completely once treatment is stopped.

  • Hyperprolactinemia (increased plasma prolactin levels) may lead to galactorrhea and gynecomastia.
  • In rare cases, allergic reactions like rash and urticaria may occur.
  • In infants, the blood-brain barrier is less developed, which may increase the risk of neurological side effects.

Pregnancy & Lactation

  • Pregnancy: Domperidone has not shown teratogenic effects in animals, but it should only be used during the first trimester if the therapeutic benefit justifies the risk. There is no evidence of increased malformation risks in humans.
  • Lactation: Domperidone is excreted in breast milk. However, its concentration in breast milk is much lower than in plasma. It is not known whether this affects the newborn, so breastfeeding is not recommended unless the expected benefits outweigh the risks.

Precautions & Warnings

  • Domperidone should be used with caution in children due to an increased risk of extrapyramidal reactions in those with an immature blood-brain barrier.
  • Since Domperidone is metabolized by the liver, it should be used with caution in patients with hepatic impairment.

Use in Special Populations

  • Infants: Domperidone should be used with extreme caution in infants as their blood-brain barrier and metabolism are still developing.
  • Liver disorders: Use with caution in patients with liver impairment.
  • Kidney disorders: In patients with severe renal insufficiency, reduce the dosing frequency to once or twice daily based on the severity.

Overdose Effects

  • Symptoms may include drowsiness, disorientation, and extrapyramidal reactions, particularly in children.
  • Treatment for overdose includes activated charcoal and close observation of the patient. Anticholinergic, antiparkinson drugs, or antihistamines with anticholinergic properties may help manage extrapyramidal reactions.
