Description
Indications
Duvent is used for the symptomatic relief of Seasonal and Perennial Allergic Rhinitis and Urticaria.

Pharmacology
Rupatadine is a long-acting, non-sedating H1-receptor antagonist. Additionally, it antagonizes platelet-activating factor (PAF). Both histamine and PAF contribute to bronchoconstriction, leading to increased vascular permeability and acting as mediators in the inflammatory process. With its dual mechanism of action, Rupatadine provides a more effective therapeutic response compared to a single antihistamine. Rupatadine also demonstrates other anti-allergic effects, including inhibiting mast cell degranulation triggered by both immunological and non-immunological stimuli, and suppressing the release of cytokines, particularly tumor necrosis factor-alpha (TNF-α) in human mast cells and monocytes.
Dosage & Administration
- Adults and adolescents (over 12 years): The recommended dose is 10 mg once daily. Rupatadine can be taken with or without food.
- Children aged 2 to 11 years:
- Children weighing 25 kg or more: 1 teaspoon (5 ml) of the oral solution once daily, with or without food.
- Children weighing between 10 kg and 25 kg: 1/2 teaspoon (2.5 ml) of the oral solution once daily, with or without food.

Interaction
- With medicines: Co-administration of Duvent 20 mg with ketoconazole or erythromycin may increase systemic exposure. Duvent should be used with caution when combined with these drugs or other CYP3A4 inhibitors. Caution is also advised when used with statins, CNS depressants, or alcohol.
- With food: Grapefruit and grapefruit juice should be avoided while using Rupatadine.
Contraindications
- Hypersensitivity to Rupatadine or any of the excipients.
Side Effects
- Common: Asthenia, dizziness, drowsiness.
- Uncommon: Increased appetite, arthralgia, back pain, impaired concentration, constipation, cough, diarrhea, dry throat, nosebleeds, fever, gastrointestinal discomfort, increased risk of infection, irritability, malaise, myalgia, nasal dryness, nausea, oropharyngeal pain, rash, thirst, vomiting, weight gain.
- Rare: Palpitations, tachycardia.
Pregnancy & Lactation
There is no clinical data on Rupatadine use during pregnancy. Pregnant women should use Rupatadine only if the potential benefits justify the risks to the infant. It is unknown if Rupatadine is excreted in breast milk. Therefore, it is not recommended during lactation unless the benefits to the mother outweigh potential risks to the infant.
Precautions & Warnings
Duvent should be used with caution in patients with known QT interval prolongation, uncorrected hypokalemia, or proarrhythmic conditions such as clinically significant bradycardia or acute myocardial ischemia. Duvent should also be used cautiously in elderly patients (65 years and older). There is no clinical experience with Duvent in patients with impaired renal or liver function, and its use is currently not recommended in these populations.
Use in Special Populations
- Elderly: Duvent should be used cautiously in elderly patients. There is no evidence to suggest a need for dose adjustments in this group.
- Children: The safety and efficacy of Duvent have not been established in patients under 12 years of age.
- Patients with renal or hepatic insufficiency: Duvent is not recommended for patients with renal or hepatic insufficiency, as no relevant clinical data is available.
Overdose Effects
Somnolence is the most common adverse effect of overdose. If accidental ingestion of high doses occurs, symptomatic treatment and necessary supportive measures should be provided.
