Indications
Electro-K is used for the following conditions:

  • Hypochloraemic alkalosis associated with hypokalemia

Pharmacology
Potassium Chloride is essential for treating hypokalemia and conditions of potassium deficiency. Potassium is the primary intracellular cation in most body tissues and is involved in various vital physiological processes. These include maintaining intracellular tonicity, nerve impulse transmission, contraction of cardiac, skeletal, and smooth muscles, as well as normal renal function.


Dosage & Administration

Oral dose:
This medication should be taken with or after meals and with a sufficient amount of fluid (water or fruit juice). For preventing hypokalemia, typical doses may reach up to 50 mmol (approximately 37.5 mL) daily. Oral treatment is appropriate in certain hypokalemia cases, with similar doses to those used for prevention being effective in mild potassium deficiency. In more severe deficiencies, higher doses may be necessary.

Parenteral dose:
Intravenous administration may be required for acute hypokalemia. One ampoule (10 mL), equivalent to 1.5 gm (20 mmol K+), can be added to 500 mL of sodium chloride or glucose intravenous infusion and administered slowly over 2 to 3 hours under professional supervision. ECG monitoring should be used in challenging cases. Frequent potassium level measurements are crucial to determine if further infusions are needed and to prevent hyperkalemia, especially in patients with renal impairment.

Interactions
Concomitant use with the following may increase the risk of hyperkalemia:

  • Angiotensin-converting enzyme inhibitors
  • Cyclosporine
  • NSAIDs
  • Beta-blockers
  • Heparin
  • Digoxin
  • Potassium-sparing diuretics

Contraindications
Electro-K should not be used in individuals with:

  • Hypersensitivity to potassium administration (e.g., hyperkalemic periodic paralysis, congenital paramyotonia)
  • Severe renal failure (even without hyperkalemia)
  • Untreated Addison’s disease
  • Hyporeninemic hypoaldosteronism
  • Acute dehydration
  • Hyperkalemia
  • Extensive cell destruction (e.g., severe burns)
  • Metabolic acidosis (should not be treated with potassium chloride in such cases)

Side Effects
Excessive potassium intake can lead to hyperkalemia, with symptoms such as:

  • Paresthesia in the extremities
  • Muscle weakness
  • Paralysis
  • Hypotension
  • Cardiac arrhythmias
  • Heart block
  • Cardiac arrest

Pain or phlebitis may occur with intravenous administration of solutions containing approximately 30 mmol or more potassium per liter. Nausea, vomiting, diarrhea, and abdominal cramps may also occur after oral administration.


Pregnancy & Lactation
As a general guideline, medications should be avoided during the first trimester of pregnancy. The risks and benefits of taking this medication during pregnancy should be carefully evaluated. There are no contraindications for breastfeeding while using potassium salts.


Precautions & Warnings
Caution should be exercised when administering Electro-K to patients with:

  • Renal or adrenal insufficiency
  • Cardiac disease
  • Acute dehydration
  • Heat cramps
  • Extensive tissue damage (e.g., severe burns)
  • Those taking potassium-sparing diuretics

Excessive use of potassium salt substitutes or concurrent use with potassium supplements can lead to potassium accumulation, especially in individuals with renal insufficiency. It’s important to monitor for other drugs that contain potassium or may cause hyperkalemia. Discontinue treatment if severe nausea, vomiting, or abdominal discomfort occurs.

Children: No special precautions are needed when used in children.
