Indications
Freeway is indicated for the treatment of:

  • Bronchial asthma
  • Bronchospasm
  • Chronic obstructive pulmonary disease (COPD)
  • Pulmonary diseases with a spastic bronchial component

Pharmacology
Doxophylline is an innovative bronchodilator that differs structurally from Theophylline due to the presence of a dioxolane group at position 7. This agent selectively inhibits phosphodiesterase 4, which relaxes bronchial smooth muscle and helps improve airflow. Unlike Theophylline, Doxophylline has a reduced affinity for adenosine A1 and A2 receptors, contributing to a more favorable safety profile. Additionally, Doxophylline has been shown to inhibit platelet-activating factor (PAF) and decrease the production of leukotrienes.

Dosage & Administration

  • For the elderly: One 200 mg tablet, two to three times per day.
  • For adults: One 400 mg tablet, two to three times per day, or as prescribed by your physician.
  • For children over 12 years: 10 ml of syrup or one 200 mg tablet, two to three times per day.
  • For children between 6-12 years: 6-9 mg/kg body weight, administered twice daily. For example, if the child weighs 10 kg, administer 3 ml (60 mg) twice daily, or as instructed by your healthcare provider.
  • If a daily dose of 400 mg is required, consider using the extended-release 400 mg tablet, once daily, or as directed by the physician.

Interaction

  • Freeway should not be taken concurrently with other xanthine-based drugs.
  • It is advisable to limit the intake of caffeine-containing beverages and foods while on this medication.
  • Caution is advised when combining Freeway with ephedrine or other sympathomimetic drugs.
  • Certain medications, such as erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccines, and propranolol, may reduce the liver clearance of xanthine derivatives, resulting in elevated drug levels.
  • Conversely, drugs like phenytoin, other anticonvulsants, and smoking may increase xanthine clearance, which might necessitate dosage adjustments.

Contraindications
Doxophylline should not be used in individuals who are hypersensitive to the drug or other xanthine derivatives. It is also contraindicated for patients with acute myocardial infarction, low blood pressure (hypotension), or during breastfeeding.

Side Effects
Patients using xanthine derivatives may experience:

  • Nausea, vomiting
  • Epigastric pain
  • Headaches, irritability
  • Insomnia
  • Tachycardia, extrasystoles
  • Increased heart rate (tachypnea)
  • Rare occurrences of hyperglycemia or albuminuria

In case of overdose, severe cardiac arrhythmias and tonic-clonic seizures may occur. These effects may indicate early intoxication. If side effects occur, discontinuation of the treatment may be necessary. If appropriate, lower doses may be resumed once the toxicity symptoms subside, under the guidance of a physician.

Pregnancy & Lactation
Animal studies have not shown any adverse effects on pre- and postnatal development. However, clinical data on the use of Doxophylline during pregnancy is limited. Therefore, the use of this medication during pregnancy should be carefully considered by a healthcare provider, weighing the potential risks and benefits. Doxophylline is contraindicated during breastfeeding.

Effects on Ability to Drive and Operate Machinery
Freeway does not affect alertness and does not impair the ability to drive or operate machinery.

Precautions & Warnings
Several factors may reduce the hepatic clearance of xanthine derivatives, leading to increased plasma levels. These factors include age, congestive cardiac failure, COPD, severe liver disease, concurrent infections, and the use of drugs like erythromycin, lincomycin, cimetidine, and others. If such conditions are present, the dosage may need to be reduced, and monitoring blood levels is recommended to ensure therapeutic levels.

Caution should be taken when administering Freeway to individuals with:

  • Cardiac disease, hypertension
  • The elderly
  • Severe hypoxemia
  • Hyperthyroidism
  • Chronic corpulmonale
  • Congestive heart failure
  • Liver disease
  • Renal impairment

In particular, patients with heart failure should use this medication with extra caution, as drug clearance is slower in these individuals, and high blood levels may persist even after discontinuation. The risk of addiction or dependence is not associated with Freeway.

Overdose Effects
An overdose may lead to severe cardiac arrhythmias and tonic-clonic seizures. These symptoms indicate possible intoxication. In such cases, treatment should be discontinued, and cardiovascular collapse should be managed symptomatically. There is no specific antidote, so supportive care is necessary.
