Indications
Racetril is recommended as an adjunct to rehydration therapy for the symptomatic management of acute secretory diarrhea.

Pharmacology
Racecadotril is a prodrug that is metabolized into its active form, thiorphan. Thiorphan inhibits enkephalinase, a peptidase found in various tissues, particularly in the small intestine’s epithelium. This enzyme typically breaks down both exogenous and endogenous peptides, including enkephalins. By protecting enkephalins from breakdown, racecadotril enhances their action at enkephalinergic synapses in the intestine, thereby reducing the hypersecretion of water and electrolytes. It works rapidly to treat diarrhea without altering the intestinal transit time and does not cause abdominal distension.
Dosage & Administration
- Adults: Start with one capsule, regardless of the time of day, followed by one capsule three times daily (preferably before meals) until diarrhea resolves.
- Children (3 months to 18 years):
- For children over 3 months, the recommended dose is 1.5 mg/kg body weight.
- If weight is less than 9 kg: 10 mg three times daily.
- If weight is 9-13 kg: 20 mg three times daily.
- If weight is 13-27 kg: 30 mg three times daily.
- If weight exceeds 30 kg: 60 mg three times daily.
Maximum treatment duration is 7 days.
How to use sachet:
- Dissolve the powder in 20 ml of pure water.
- Stir well and drink the entire suspension immediately after dissolving.

Interaction
No significant interactions with other active substances have been reported. Co-administration with Loperamide or Nifuroxazide does not alter Racetril’s pharmacokinetics.
Contraindications
Racetril is contraindicated in individuals with hypersensitivity to any of its ingredients.
Side Effects
Possible side effects include headache, rash, and erythema.
Pregnancy & Lactation
Due to insufficient clinical data, Racetril is not recommended during pregnancy or breastfeeding.
Precautions & Warnings
Racetril should not replace standard rehydration protocols. Caution is advised for patients with renal or hepatic impairments, as data in these cases is limited.
Overdose Effects
No overdose cases have been reported. In clinical trials, doses up to 2 g (20 times the standard dose) have been administered without harmful effects.
