Indications
Seebri inhalation capsule is indicated for once-daily maintenance bronchodilator treatment to relieve symptoms in patients with chronic obstructive pulmonary disease (COPD).

Pharmacology
Glycopyrronium is a long-acting, specific antimuscarinic agent (also known as an anticholinergic). It has a similar affinity for M1 to M5 muscarinic receptors, with M3-receptor inhibition in the airways leading to smooth muscle relaxation. Its high potency and slow receptor dissociation contribute to significant and long-acting bronchodilation in COPD patients.

Dosage & Administration

  • The recommended dose is one inhalation capsule (50 micrograms) once daily using the designated inhalation device.
  • This should be administered at the same time each day via the oral inhalation route.
  • If a dose is missed, the patient should take it as soon as they remember, but should not exceed one dose per 24 hours.

Interaction

  • No specific studies have been conducted on the co-administration of this inhalation capsule with other inhaled anticholinergic drugs; therefore, such use is not recommended.
  • Seebri has been used with other COPD treatments, including sympathomimetic bronchodilators, methylxanthines, oral and inhaled steroids, without clinical evidence of drug interactions.
  • Co-administration with indacaterol (a β2-adrenergic agonist) under steady-state conditions does not alter the pharmacokinetics of either drug.

Contraindications
Seebri inhalation capsule is contraindicated in patients with:

  • Hypersensitivity to glycopyrronium or any excipients in the formulation.

Side Effects
Common side effects:

  • Dry mouth, cough, nasopharyngitis, vomiting, musculoskeletal pain, neck pain, diabetes mellitus, gastroenteritis, insomnia.

Less common side effects:

  • Paradoxical bronchospasm, dysphonia, angioedema, hypersensitivity, pruritus, sinus congestion, throat irritation.

In elderly patients (above 75 years):

  • Higher incidence of urinary tract infections and headaches.

Pregnancy & Lactation

  • Pregnancy: There are no well-controlled studies on the use of Seebri in pregnant women with COPD. It should only be used if the potential benefit justifies the possible risk to the fetus.
  • Lactation: It is unknown whether Glycopyrronium Bromide or its metabolites are excreted in human milk. Use in breastfeeding women should be considered with caution, weighing the benefits against potential risks to the infant.
  • Fertility: Animal studies do not indicate any fertility concerns for males or females.

Precautions & Warnings

  • Seebri is a long-term maintenance therapy and not for acute bronchospasm relief.
  • Not suitable for patients under 18 years of age.
  • Use with caution in patients with:
    • Narrow-angle glaucoma
    • Urinary retention
  • If allergic reactions occur, such as angioedema (swelling of the face, tongue, lips, or difficulty breathing), hives, or skin rash, discontinue use immediately and seek alternative treatment.
  • Paradoxical bronchospasm may occur, which can be life-threatening. If this happens, discontinue use and initiate alternative therapy.
  • In patients with severe renal impairment (eGFR <30 mL/min/1.73m²) or end-stage renal disease requiring dialysis, Seebri should only be used if the benefits outweigh the potential risks.

Overdose Effects

  • High doses may cause anticholinergic symptoms, requiring symptomatic treatment.
  • In COPD patients, repeated administration of 100-200 mcg daily for 28 days has been well tolerated.
  • Accidental oral ingestion of Seebri capsules is unlikely to cause acute toxicity.

Storage Conditions

  • Do not swallow this capsule—use only with the designated inhalation device.
  • Insert the Easycap into the Easyhaler for best performance.
  • Remove the capsule from the blister immediately before use, as exposure to moisture may affect integrity.
