Valenty is used for the treatment of erectile dysfunction (ED).

Pharmacology
Vardenafil is a selective inhibitor of phosphodiesterase type 5 (PDE5) that specifically targets cyclic guanosine monophosphate (cGMP). The process of penile erection is initiated by the relaxation of smooth muscles in the corpus cavernosum and the surrounding arterioles. Upon sexual stimulation, nitric oxide is released in the corpus cavernosum, activating the enzyme guanylate cyclase, which leads to an increase in cGMP levels in the smooth muscle cells. This rise in cGMP promotes smooth muscle relaxation, allowing increased blood flow into the penis, resulting in an erection. By inhibiting PDE5, Vardenafil helps to increase the levels of cGMP, improving erectile function. It is important to note that sexual stimulation is necessary to trigger the local release of nitric oxide; therefore, PDE5 inhibition does not cause an erection without sexual arousal.

Dosage & Administration
The recommended initial dose is 10 mg, taken orally approximately 60 minutes before sexual activity. The dosage can be adjusted based on efficacy and side effects, with a maximum dose of 20 mg or a reduced dose of 5 mg as needed. The maximum recommended dosing frequency is once daily. Vardenafil can be taken with or without food.

For geriatric patients (over 65 years old), a starting dose of 5 mg is recommended.

For patients with renal impairment, no dosage adjustment is needed.

For patients with hepatic impairment, a starting dose of 5 mg is recommended, and the maximum dose should not exceed 10 mg. Vardenafil should not be used in patients with severe hepatic impairment.

Drug Interactions
Valenty may enhance the blood pressure-lowering effects of antihypertensive medications. In clinical studies, a single dose of 20 mg Valenty resulted in a mean reduction of 7 mmHg in systolic blood pressure and 8 mmHg in diastolic blood pressure. Caution is advised when co-administering with other vasodilators, including alpha blockers. Valenty does not amplify the hypotensive effects of alcohol. CYP3A4 inhibitors (such as ketoconazole, indinavir, ritonavir, and erythromycin) may reduce the clearance of Valenty from the body.

Contraindications
Vardenafil should not be taken with nitrates (either regularly or intermittently) or nitric oxide donors, as it may enhance the hypotensive effects of nitrates. It is also contraindicated in individuals who have shown hypersensitivity to Vardenafil or any of the components of this medication.

Side Effects
Common side effects of Valenty include headache, facial flushing, nasal congestion, indigestion, upset stomach, dizziness, or back pain.

Pregnancy & Lactation
Vardenafil is not intended for use in women.

Precautions & Warnings
Physicians should carefully evaluate the cardiovascular health of patients, as sexual activity can carry a degree of cardiac risk. For men who have been advised against sexual activity due to their cardiovascular condition, Valenty should generally not be used.

Overdose Effects
The highest dose studied in clinical trials was 120 mg in a single dose, where the majority of participants experienced blurred vision and back pain. Single doses of up to 80 mg and daily doses up to 40 mg for 4 weeks did not result in serious adverse effects. In case of an overdose, immediate medical attention is required.
