Description
Indications
Vestar MR is indicated for adults as an additional therapy to treat stable angina pectoris in patients who do not respond well to or are intolerant of first-line antianginal treatments.

Pharmacology
Trimetazidine Dihydrochloride is the first 3-keto acyl CoA thiolase inhibitor (KAT), a metabolic anti-ischemic agent with proven effectiveness in coronary patients. It works by inhibiting the fatty acid oxidation pathway via the inhibition of the 3-keto acyl CoA thiolase enzyme. This shifts metabolism towards glucose utilization, which is a more efficient energy-producing process, requiring less oxygen to generate more energy. This process enhances cardiac function and prevents the accumulation of lactic acid, which is responsible for triggering angina pectoris.
Dosage & Administration
The usual dosage of Trimetazidine is 35 mg twice a day or 20 mg three times a day, taken with meals. The treatment’s effectiveness should be reviewed after three months. If there is no significant response, discontinuing the medication is recommended.

Interaction
No significant drug interactions have been reported with Vestar MR. It is especially safe when used in combination with beta-blockers, calcium channel blockers, nitrates, heparin, cholesterol-lowering agents, and digitalis preparations.
Contraindications
Trimetazidine should not be used by patients who are allergic to the active ingredient or any of the excipients. It is also contraindicated in individuals with Parkinson’s disease, tremors, restless legs syndrome, movement disorders, or severe renal impairment.
Side Effects
Vestar MR is generally well tolerated. However, common side effects may include dizziness, headache, abdominal discomfort, diarrhea, indigestion, nausea, vomiting, skin rashes, itching, hives, and weakness.
Pregnancy & Lactation
There is insufficient data on the use of Trimetazidine during pregnancy. Animal studies have not shown significant reproductive toxicity, but it is recommended to avoid Trimetazidine during pregnancy as a precautionary measure. It is also unknown whether Trimetazidine is excreted in breast milk, and a risk to infants cannot be ruled out. Therefore, Trimetazidine should not be used while breastfeeding.

Vestar MR is not a cure for angina attacks nor is it an appropriate treatment for unstable angina or myocardial infarction.
