Description
Indications
Vifas is primarily indicated for the treatment of allergic rhinitis and chronic idiopathic urticaria (hives). It is effective in alleviating symptoms associated with both seasonal and perennial allergic rhinitis in adults and children aged 12 years and older. The symptoms include sneezing, runny nose, watery eyes, itching of the nose, throat, and palate, and itchy or red eyes. It is also used to relieve the discomfort caused by chronic hives, including itching and skin rashes.

Pharmacology
The H1 histamine receptor plays a central role in allergic reactions and hypersensitivity. When exposed to allergens, mast cells and basophils degranulate, releasing histamine and other inflammatory mediators. Histamine binds to H1 receptors, leading to the release of cytokines like interleukins, which cause inflammation and symptoms such as itching, runny nose, and watery eyes.
Fexofenadine, the active ingredient in Vifas, acts as an “inverse agonist” at the H1 receptor. This means it binds to the receptor, stabilizing it in an inactive form, which prevents histamine-induced activation and its downstream effects. Fexofenadine is highly selective for H1 receptors and does not display significant antidopaminergic, antiserotonergic, anticholinergic, sedative, or adrenergic blocking activities. Additionally, Fexofenadine does not cross the blood-brain barrier, reducing the likelihood of central nervous system effects.
Dosage & Administration
For Allergic Rhinitis:

- Tablet: 60 mg twice daily, 120 mg once daily, or 180 mg once daily.

Interaction
Vifas does not undergo significant hepatic biotransformation, thus avoiding interactions via liver-based metabolism. However, coadministering Vifas with erythromycin or ketoconazole can increase fexofenadine plasma levels by 2 to 3 times. Despite this, no adverse effects on the QT interval or an increase in side effects have been observed. Fexofenadine does not interact with omeprazole, but taking it with antacids containing aluminum and magnesium hydroxide may reduce its absorption. It is recommended to space out the administration of antacids by at least 2 hours.
Contraindications
Vifas should not be used in patients who have a known hypersensitivity to Fexofenadine Hydrochloride or any of its ingredients.
Side Effects
Common Side Effects:
- Nervous system: headache, dizziness, drowsiness.
- Gastrointestinal: nausea.
- General: fatigue.
Less common but serious side effects:
- Hypersensitivity reactions such as angioedema, chest tightness, breathing difficulties, skin flushing, and anaphylaxis.
- Cardiac issues: tachycardia, palpitations.
- Gastrointestinal: diarrhea.
- Skin: rash, urticaria, and itching.
Pregnancy & Lactation
There is no adequate data on the use of Fexofenadine Hydrochloride during pregnancy. While limited animal studies show no direct harmful effects, it should only be used during pregnancy if the potential benefits outweigh the risks. Fexofenadine is not recommended for breastfeeding mothers, as it may pass into breast milk. There is no data available on its effect on human fertility, but animal studies have shown no impact on fertility.
Precautions & Warnings
Vifas should be used with caution in elderly patients and those with renal or hepatic impairment. Antihistamines, including Fexofenadine, have been linked with tachycardia and palpitations in patients with cardiovascular disease.
Effect on ability to drive and use machinery:
Based on its pharmacodynamic profile, Vifas is unlikely to impair the ability to drive or operate machinery. However, it is advisable to assess individual sensitivity before engaging in tasks requiring concentration.

Renal and Hepatic Impairment:
The pharmacokinetics of Fexofenadine are altered in patients with renal impairment, so a dose of 60 mg once daily is recommended for those with decreased renal function. Moderate to severe hepatic disease does not significantly affect its pharmacokinetics. -
Elderly Patients:
Fexofenadine‘s bioavailability is increased in individuals over 65, but adverse events are similar to those seen in patients under 65.
Overdose Effects
Overdose may result in dizziness, drowsiness, fatigue, and dry mouth. No significant adverse reactions have been noted with doses up to 800 mg. Standard measures should be taken to remove any unabsorbed medication, and symptomatic treatment is recommended. Hemodialysis is not effective in removing Fexofenadine from the bloodstream.
