Description

Indications:
This solution is used for the treatment of reversible bronchospasm associated with obstructive airway diseases. It is particularly effective in patients requiring more than a single bronchodilator.

Description:
This inhalation solution combines two active ingredients: Salbutamol Sulfate and Ipratropium Bromide.

• Salbutamol Sulfate is a beta2-adrenergic agent that relaxes the airway smooth muscle, providing protection against bronchoconstrictor challenges from the trachea to the terminal bronchioles.
• Ipratropium Bromide is an anticholinergic that inhibits vagally mediated reflexes by blocking acetylcholine’s action, primarily providing local bronchodilation.

The synergy of these two bronchodilators provides superior bronchodilation compared to each agent alone, with no increased risk of adverse effects.

Pharmacology:
Ipratropium Bromide has low systemic absorption and primarily local effects in the lungs, with minimal penetration into the bloodstream. Salbutamol Sulfate is absorbed rapidly and is excreted unchanged in the urine. Co-nebulization of both drugs does not increase systemic absorption, ensuring the combined benefits of bronchodilation remain lung-specific.

Dosage:

• Adults (including elderly): One 3 ml ampoule in a nebulizer, four times a day. Additional treatments may be used if necessary.
• Children: Dosage must be determined by a doctor.
• Administration Instructions: Twist off the top of the ampoule, add the solution to the nebulizer, and follow the physician’s instructions for dilution if required.

Interaction:
The use of other beta-mimetics, anticholinergics, or xanthine derivatives can enhance side effects, particularly hypokalaemia, which may increase susceptibility to arrhythmias. Beta-blockers should be avoided, and careful monitoring is necessary when using in combination with other medications.

Contraindications:
Not recommended for patients with hypertrophic obstructive tachyarrhythmia or hypersensitivity to atropine or its derivatives.

Side Effects:
Common side effects include muscle tremors, nervousness, tachycardia, dizziness, and palpitations. Potentially serious side effects may include hypokalaemia, hypertension, arrhythmias, and ocular issues (mydriasis, increased intraocular pressure). Other less common side effects include dry mouth, gastrointestinal disturbances, and urinary retention.

Pregnancy & Lactation:
This medication should be used during pregnancy only if the potential benefit justifies the risk. It is excreted in breast milk, so caution should be exercised when used by nursing mothers.
