Description
Indications: Xalcort is used for the treatment of various conditions, including:
- Allergic Reactions: Anaphylaxis, asthma, severe hypersensitivity reactions
- Autoimmune Disorders: Rheumatoid arthritis, juvenile chronic arthritis, polymyalgia rheumatica, systemic lupus erythematosus, and others
- Skin Conditions: Pemphigus, bullous pemphigoid, pyoderma gangrenosum
- Nephrotic Syndrome: Minimal change nephrotic syndrome, acute interstitial nephritis
- Other Conditions: Rheumatic carditis, ulcerative colitis, Crohn’s disease, uveitis, optic neuritis, autoimmune haemolytic anaemia, idiopathic thrombocytopenic purpura
- Cancer and Immune Suppression: Acute and lymphatic leukaemia, malignant lymphoma, multiple myeloma, and immune suppression in transplantation
Pharmacology: Xalcort contains Deflazacort, a corticosteroid that provides anti-inflammatory effects by inhibiting the enzyme Phospholipase A2, crucial for prostaglandin synthesis. Additionally, it decreases the release of certain immune system chemicals, providing immunosuppressive effects.
Dosage and Administration:
- Adults: The dose depends on the severity and condition. For acute disorders, up to 120 mg/day may be required initially. For maintenance, doses range from 3-18 mg/day.
- Children: The recommended dosage varies between 0.25-1.5 mg/kg/day depending on the condition. Gradual titration is necessary.
- Deflazacort Withdrawal: Gradual dose reduction is required after prolonged corticosteroid use to avoid relapse.
Interactions:
- Liver enzyme inducers (e.g., rifampicin, phenobarbitone) may require a dose increase.
- Liver enzyme inhibitors (e.g., ketoconazole) may allow for a dose reduction.
Contraindications:
- Hypersensitivity to Deflazacort or any components
- Live virus immunizations
- Severe infections or systemic fungal infections
Side Effects: Possible side effects include gastrointestinal disturbances, musculoskeletal issues, fluid and electrolyte disturbances, impaired healing, and hypersensitivity reactions. Rare effects include myocardial rupture, thromboembolism, and ocular issues.
Pregnancy and Lactation:
- Pregnancy: Use only if the benefits outweigh the risks, as it may cause intrauterine growth retardation.
- Lactation: Excreted in breast milk in small amounts; doses up to 50 mg/day are unlikely to harm the infant.
Precautions and Warnings:
- Caution is advised in patients with cardiac disease, hypertension, diabetes, osteoporosis, or liver/renal issues.
- Regular monitoring for side effects is recommended during long-term use.
