Description
Indications:
Xalcort is indicated for a variety of conditions, including but not limited to:
- Anaphylaxis & severe hypersensitivity reactions
- Rheumatologic conditions such as rheumatoid arthritis, juvenile chronic arthritis, polymyalgia rheumatica, and systemic lupus erythematosus
- Autoimmune disorders like dermatomyositis, mixed connective tissue disease, and pemphigus
- Nephrology: Minimal change nephrotic syndrome and acute interstitial nephritis
- Gastrointestinal disorders such as ulcerative colitis and Crohn’s disease
- Uveitis and optic neuritis
- Hematologic conditions: Autoimmune hemolytic anemia and idiopathic thrombocytopenic purpura
- Cancer treatment: Acute and lymphatic leukemia, lymphoma, and multiple myeloma
- Post-transplantation immunosuppression

Pharmacology:
Xalcort (Deflazacort) works by inhibiting the phospholipase A2 enzyme, reducing the synthesis of prostaglandins and decreasing the release of chemicals that are important in the immune response. This mechanism provides both anti-inflammatory and immunosuppressive actions, useful in various inflammatory and autoimmune conditions.
Dosage & Administration:
- Adults: Starting dose may be up to 120 mg/day for acute disorders, with maintenance typically ranging from 3-18 mg/day, depending on the condition.
- Children: Dosage ranges from 0.25-1.5 mg/kg/day, adjusted based on the specific condition.
- Tapering off: Gradual reduction is recommended in patients who have been on high doses for prolonged periods to avoid withdrawal symptoms.
Interaction:
- Increased metabolism may occur when Xalcort is used with liver enzyme inducers (e.g., rifampicin, phenytoin), requiring dose adjustments.
- Liver enzyme inhibitors (e.g., ketoconazole) may reduce the maintenance dose.
Contraindications:
- Known hypersensitivity to Deflazacort or any ingredients in the formulation.
- Patients receiving live virus vaccinations.
Side Effects:
- Common side effects may include GI disturbances, musculoskeletal symptoms, skin issues like acne or atrophy, and fluid/electrolyte imbalances.
- Serious effects could include thromboembolism or myocardial rupture, especially post-MI.
Pregnancy & Lactation:
- Pregnancy: Xalcort should only be used if the benefits outweigh the risks to the fetus.
- Lactation: Deflazacort is excreted in breast milk, but at lower concentrations, making it generally considered safe at therapeutic doses.
Precautions & Warnings:
Use cautiously in patients with cardiovascular disease, diabetes, active infections, or GI conditions like ulcers. Monitor for electrolyte disturbances and potential psychiatric effects. Special attention is needed in patients with liver or renal impairment.
