Description

Indications:
Xeldrin is indicated for the treatment of the following conditions:

  • Gastric and duodenal ulcers
  • NSAID-associated duodenal and gastric ulcers
  • Prophylaxis in patients with a history of NSAID-associated duodenal and gastric ulcers
  • Gastro-esophageal reflux disease (GERD)
  • Long-term management of acid reflux disease
  • Acid-related dyspepsia
  • Severe ulcerating reflux esophagitis
  • Prophylaxis of acid aspiration during general anesthesia
  • Zollinger-Ellison syndrome
  • Helicobacter pylori-induced peptic ulcer

Dosage and Administration:

  • Oral:

    • Benign gastric and duodenal ulcer: 20 mg once daily for 4 weeks in duodenal ulceration, 8 weeks in gastric ulceration; increase dose to 40 mg daily for severe or recurrent cases.
    • NSAID-associated ulcers: 20 mg once daily for 4 weeks, continued for a further 4 weeks if not fully healed.
    • Gastro-esophageal reflux disease (GERD): 20 mg once daily for 4 weeks; increase to 40 mg if necessary, with a maintenance dose of 20 mg daily.
    • Zollinger-Ellison syndrome: Initially 60 mg once daily, adjust dose as needed (up to 120 mg daily).
    • Helicobacter pylori eradication: 20 mg twice daily in combination with antimicrobial agents.
  • Intravenous Injection:

    • Prophylaxis of acid aspiration: 40 mg one hour before surgery.
    • Reflux esophagitis and ulcers: 40 mg once daily if oral administration is inappropriate.
    • Zollinger-Ellison syndrome: Initial dose of 60 mg, adjust as needed.

Pharmacology:
Omeprazole, the active ingredient in Xeldrin, is a proton pump inhibitor that reduces gastric acid secretion. It works by inhibiting the hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in gastric parietal cells. The onset of action occurs within one hour, with maximum effect in two hours. Its effect lasts up to 72 hours, and secretory activity gradually returns over 3-5 days after discontinuation.

Side Effects:
Xeldrin is generally well-tolerated, but some side effects may include:

  • Skin rash, urticaria, pruritus, photosensitivity
  • Diarrhea, headache, constipation, nausea/vomiting
  • Reversible mental confusion, dizziness, insomnia
  • Gastrointestinal discomfort (e.g., flatulence, abdominal pain)
  • Rare side effects: hepatic failure, anaphylaxis, interstitial nephritis

Precautions & Warnings:

  • Avoid concomitant use with clopidogrel, as it may reduce the pharmacological activity of clopidogrel.
  • May increase the risk of osteoporosis-related fractures with long-term use.
  • Caution in patients with liver impairment.
  • Consult a registered physician before use.

Pregnancy & Lactation:
Xeldrin is considered safe for use during pregnancy based on epidemiological studies. The effects on breastfeeding infants are unknown; hence, discontinuation of breastfeeding is recommended if Omeprazole is essential.
